Pharmacology
- Triazole antifungal agent
- Potent inhibitor of fungal cytochrome P-450 dependent enzymes
- Responsible for the synthesis of ergosterol in fungal cell membrane
Dosage & Administration
- Different doses for various indications
- Dosage adjustment for children based on weight
- Dosage adjustment for elderly and patients with renal impairment
- Specific recommendations for multiple dose therapy in patients with renal impairment
Interaction
- Increased prothrombin time after warfarin administration
- Prolonged serum half-life of concomitantly administered oral sulphonylureas
- Increased plasma concentrations of fluconazole with co-administration of hydrochlorothiazide
- Possible increase in phenytoin levels
- Decreased AUC and shorter half-life of fluconazole with rifampicin administration
- Potential effects on combined oral contraceptives
- Slow increase in cyclosporin concentrations with fluconazole
- No impairment of fluconazole absorption with co-administration of food, cimetidine, antacids, or following total body irradiation
- Decreased plasma clearance of theophylline with fluconazole administration
Contraindications
- Known hypersensitivity to Fluconazole or related triazole compounds
Side Effects
- Common gastrointestinal symptoms
- Rarely encountered rash and anaphylaxis
Pregnancy & Lactation
- Adverse fetal effects seen in animals at high dose levels
- Not recommended for use unless adequate contraception is employed
Precautions & Warnings
- Abnormalities of hepatic, renal, haematological, and biochemical function tests observed in some patients
- Rare occurrence of hepatic necrosis
- Rare development of severe cutaneous reactions, particularly in AIDS patients
- Monitoring and discontinuation of therapy if rash develops
- Not recommended for use during lactation
- Unlikely to impair a patient's ability to drive or use machinery
Use in Special Populations
- Normal dose for elderly if no evidence of renal impairment
- Dosage adjustments for patients with renal impairment based on creatinine clearance
Overdose Effects
- Supportive measures and symptomatic treatment recommended
- Potential use of gastric lavage if necessary
- Forced volume diuresis may increase elimination rate
- Haemodialysis decreases plasma levels
