Indications

  • Treatment of acute clinical manifestations of Herpes simplex virus in immunocompromised patients
  • Treatment of severe primary or non-primary genital herpes in immune competent patients
  • Treatment of Varicella zoster virus infection in immunocompromised patients
  • Treatment of Herpes zoster (shingles) in immune competent patients
  • Treatment of Herpes simplex encephalitis

Pharmacology

Interferes with DNA synthesis and inhibits viral replication for Herpes simplex virus and Varicella zoster virus

Dosage

  • 5 mg/kg every 8 hours for Herpes simplex infection
  • 5 mg/kg every 8 hours for very severe Herpes zoster infection
  • 10 mg/kg every 8 hours for Varicella zoster infection
  • 10 mg/kg every 8 hours for Herpes simplex encephalitis

Administration

  • Administered for 5-7 days in most infections and at least 10 days for Herpes simplex encephalitis
  • Should be administered by slow intravenous infusion over a one-hour period
  • Can be injected directly into a vein or further diluted for administration by infusion

Interaction

Co-administration of probenecid or diuretics can affect plasma levels of the drug

Hypersensitivity to Acyclovir or Valacyclovir

Pregnancy & Lactation

Pregnancy category B; should be used in pregnancy only if benefits outweigh risks

Precautions & Warnings

  • Intended for intravenous infusion only
  • Reconstituted infusion should not be administered by mouth
  • Should be given over a period of at least one hour to avoid renal tubular damage

Use in Special Populations

  • Pediatric use: Dose calculated on the basis of body surface area
  • Geriatric use: No specific data available

Overdose Effects

  • Elevations of serum creatinine and blood urea nitrogen
  • Neurological effects like confusion, seizures, and coma
  • Adequate hydration essential to reduce the possibility of crystal formation in urine
