Description
- Dopamine antagonist medication for treating nausea and vomiting
- Gastrointestinal motility stimulant
- Blocks dopamine receptors in the Chemoreceptor Trigger Zone (CTZ) and the stomach
- Restores normal motility and tone of the upper gastrointestinal tract
- Facilitates gastric emptying and enhances antral and duodenal peristalsis
- Regulates contraction of the pylorus
- Increases esophageal peristalsis and lower esophageal sphincter pressure
- Does not affect the mental alertness
- Rapid barium transit in follow-through radiological studies

Dosage & Administration
- For adults: 10-20 mg (1-2 tablets or 10-20 ml suspension) every 6-8 hours daily
- Maximum daily dose for adults is 80 mg
- Children's dosage is based on body weight and age

Pharmacology
- Blocks dopamine receptors in the Chemoreceptor Trigger Zone (CTZ) and stomach
- Gastroprokinetic action based on blocking effect of dopamine receptors
- Weak penetration across the blood-brain barrier
- Restores normal motility and tone of the upper gastrointestinal tract

Interaction
- Antagonized by anticholinergic drugs
- Should not be given simultaneously with antacids and antisecretory drugs
- Metabolism primarily through CYP3A4 pathway
- Inhibited by drugs such as azole antifungals and macrolide antibiotics
Contraindications
- Hypersensitivity to the drug
- Neonates
- Gastrointestinal hemorrhage, mechanical obstruction or perforation
- Prolactin releasing pituitary tumor (prolactinoma)
Side Effects
- Transient intestinal cramps
- Rare extrapyramidal phenomena
- Possible increase in plasma prolactin level
- Rare allergic reactions such as rash and urticaria

- Use during first trimester if justified by therapeutic benefit
- Excreted in breast milk, caution advised during nursing
