Dosage Instructions
- Should be taken 15-30 minutes before meals
- Adults: 10-20 mg (1-2 tablets), every 6-8 hours daily
- Children: 2-4 ml suspension/10 kg body weight, every 6-8 hours daily
Recommended Usage
- Adults: For dyspeptic symptoms - 10-20 mg, every 6-8 hours daily
- Children: For dyspeptic symptoms - 0.2-0.4 mg/kg body weight
- Adults: In acute nausea and vomiting - 20 mg, every 6-8 hours daily
- Children: In acute nausea and vomiting - 0.2-0.4 mg/kg body weight
- Rectal use: Adults - 30-60 mg every 4-8 hours
- Rectal use: Children - Maximum daily dose is 30 mg for 10 to 25 kg
Pharmacology
- Dopamine antagonist
- Blocks dopamine receptors in the ChemoreceptorTrigger Zone (CTZ) and stomach
- Restores normal motility and tone of the upper gastrointestinal tract
- Facilitates gastric emptying
- Enhances antral and duodenal peristalsis
Contraindications
- Known hypersensitivity to the drug
- Neonates
- Gastrointestinal stimulation might be dangerous
Interaction
- Anticholinergic drugs may antagonize the effect
- Antacids and antisecretory drugs should not be given simultaneously
- Inhibition of CYP3A4 enzyme may increase plasma levels
Side Effects
- Transient intestinal cramps
- Extrapyramidal phenomena in rare cases
- Increase in plasma prolactin level
- Rare cases of allergic reactions such as rash and urticaria
Pregnancy & Lactation
- Use during first trimester if justified by therapeutic benefit
- Excreted in breast milk, concentration in breast milk is 4 times lower than plasma
- Nursing not recommended unless benefits outweigh potential risks
Precautions
- Use with caution in children due to risk of extra-pyramidal reactions
- Highly metabolized in the liver, caution in patients with hepatic impairment
Overdose Effects
- Symptoms may include drowsiness, disorientation, and extrapyramidal reactions
- Administration of activated charcoal recommended
- Anticholinergic or antihistamines with anticholinergic properties may be helpful
