Indications
- Dyspeptic symptom complex

Pharmacology
- Dopamine antagonist
- Blocks dopamine receptors
- Gastroprokinetic action
- Weak penetration across blood-brain barrier
- Restores normal motility and tone of upper gastrointestinal tract
- Facilitates gastric emptying
- Increases esophageal peristalsis
Dosage & Administration
- Taken 15-30 minutes before meals
- Usual recommended oral dose for adults: 10-20 mg every 6-8 hours
- Usual recommended oral dose for children: 2-4 ml suspension/10 kg body weight every 6-8 hours
- Maximum dose for adults: 80 mg daily
- Maximum period of treatment is 12 weeks
Interaction
- Antagonizes antidyspeptic effect of anticholinergic drugs
- Should not be given simultaneously with antacids and antisecretory drugs
- Main metabolic pathway is through CYP3A4
- Concomitant use of CYP3A4 inhibitors may result in increased plasma levels
- Influences the absorption of orally administered drugs
- Does not potentiate action of neuroleptics
- Suppresses unwanted peripheral effects of dopaminergic agonists
Contraindications
- Known hypersensitivity to the drug
- Neonates
- Gastrointestinal hemorrhage
- Mechanical obstruction or perforation
- Prolactin releasing pituitary tumor (prolactinoma)
Side Effects
- Transient intestinal cramps
- Extrapyramidal phenomena rare in young children
- Increase in plasma prolactin level
- Rare cases of hyperprolactinaemia resulting in neuroendocrinological phenomena such as galactorrhoea and gynaecomastia
- Rare allergic reactions such as rash and urticaria
Pregnancy & Lactation
- Should be used during the first trimester of pregnancy with caution if justified by anticipated therapeutic benefit
- No evidence of increase in the risk of malformations in humans
- Excreted in breast milk of lactating rats
- Concentrations in breast milk 4 times lower than corresponding plasma concentrations
- Nursing not recommended unless expected benefits outweigh potential risk
- Does not affect mental alertness
Precautions & Warnings
- Use with caution in children due to increased risk of extra-pyramidal reactions
- Use with caution in patients with hepatic impairment
Use in Special Populations
- Caution in infants due to immature blood-brain barrier
- Use with caution in liver disorders
- Use with caution in kidney disorders
Overdose Effects
- Symptoms may include drowsiness, disorientation, extrapyramidal reactions
- Administration of activated charcoal recommended
- Close observation of patients recommended
- Anticholinergic, antiparkinson drugs or antihistamines with anticholinergic properties may be helpful in controlling extrapyramidal reactions
