Indications
- Relief of dyspeptic symptom complex
- Treatment of acute nausea and vomiting
- Used in Parkinson's disease for dopamine-agonist induced nausea and vomiting
- Speeding barium transit in radiological studies
Mode of Action
Dopamine antagonist, primarily blocking receptors in the Chemoreceptor Trigger Zone (CTZ) and stomach
Pharmacological Actions
- Restores normal motility and tone of the upper gastrointestinal tract
- Facilitates gastric emptying
- Enhances antral and duodenal peristalsis
- Regulates contraction of the pylorus
- Increases esophageal peristalsis
- Enhances lower esophageal sphincter pressure
Dosage & Administration
- Adults: 10-20 mg every 6-8 hours daily, maximum 80 mg daily
- Children: 2-4 ml suspension/10 kg body weight or 0.4-0.8 ml paediatric drops/10 kg body weight every 6-8 hours daily
Interaction
- Concomitant administration with anticholinergic drugs may antagonize its effect
- Antacids and antisecretory drugs should not be given simultaneously
- Concomitant use of drugs that significantly inhibit CYP3A4 may increase plasma levels of domperidone
- May influence absorption of orally administered drugs, particularly sustained release or enteric coated formulations
- Does not potentiate action of neuroleptics
- Suppresses unwanted peripheral effects of dopaminergic agonists
Contraindications
- Known hypersensitivity
- Neonates
- Gastrointestinal hemorrhage, mechanical obstruction or perforation
- Prolactin releasing pituitary tumor (prolactinoma)
Side Effects
- Transient intestinal cramps
- Rare extrapyramidal phenomena
- Possible increase in plasma prolactin level
- Rare allergic reactions such as rash and urticaria
Pregnancy & Lactation
- Should only be used during the first trimester if justified by therapeutic benefit
- Excreted in breast milk, caution advised
Precautions & Warnings
- Caution in children due to risk of extra-pyramidal reactions
- Caution in patients with hepatic impairment
Use in Special Populations
- Infants: Caution and close medical supervision due to immature blood-brain barrier
- Liver Disorders: Caution due to high metabolism in the liver
- Kidney Disorders: Dosing frequency may need to be reduced in severe renal insufficiency
Overdose Effects
- Drowsiness
- Disorientation
- Extrapyramidal reactions
