Pharmacology
- Dopamine antagonist
- Blocks dopamine receptors in CTZ and stomach
- Gastroprokinetic action
- Restores normal motility and tone of upper gastrointestinal tract
- Facilitates gastric emptying
- Enhances antral and duodenal peristalsis
- Regulates contraction of the pylorus
- Increases esophageal peristalsis
- Regulates contraction of the lower esophageal sphincter
DosageAndAdministration
- Take 15-30 minutes before meals
- Usual recommended oral dose for adults: 10-20 mg every 6-8 hours daily
- Maximum dose for adults: 80 mg daily
- Usual recommended dose for children: 2-4 ml suspension/10 kg body weight every 6-8 hours daily
- Maximum period of treatment for acute nausea and vomiting: 12 weeks
Interactions
- Antagonizes antidyspeptic effect of anticholinergic drugs
- Low oral bioavailability when given simultaneously with antacids and antisecretory drugs
- Main metabolic pathway is through CYP3A4
- Inhibition of CYP3A4 by drugs like azole antifungals, macrolide antibiotics, HIV protease inhibitors, etc.
- Does not potentiate action of neuroleptics
- Suppresses unwanted peripheral effects of dopaminergic agonists
Contraindications
- Known hypersensitivity to the drug
- Neonates
- Gastrointestinal hemorrhage
- Mechanical obstruction or perforation
- Prolactin releasing pituitary tumor (prolactinoma)
SideEffects
- Transient intestinal cramps
- Rare extrapyramidal phenomena
- Possible increase in plasma prolactin level
- Rare allergic reactions such as rash and urticaria
PregnancyAndLactation
- Use during first trimester justified by anticipated therapeutic benefit
- No evidence of increase in risk of malformations in humans
- Excreted in breast milk of lactating rats
- Domperidone concentrations in breast milk 4 times lower than corresponding plasma concentrations
- Not recommended for nursing mothers unless expected benefits outweigh potential risk

- Use with caution in patients with hepatic impairment
