Indications

  • Gastric and duodenal ulcer
  • NSAID-associated duodenal and gastric ulcer
  • Gastro-esophageal reflux disease
  • Acid-related dyspepsia
  • Severe ulcerating reflux esophagitis
  • Prophylaxis of acid aspiration during general anesthesia
  • Zollinger-Ellison syndrome
  • Helicobacter pylori-induced peptic ulcer

Pharmacology

  • Inhibitor of gastric acid secretion
  • Blocker of hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system
  • Onset of antisecretory effect within one hour
  • Maximum effect within two hours
  • Inhibition of secretion lasts up to 72 hours

Dosage

  • Oral administration
  • 20 mg once daily for 4 weeks in duodenal ulceration
  • 8 weeks in gastric ulceration
  • Maintenance dose for recurrent duodenal ulcer
  • In prevention of relapse in duodenal ulcer
  • 10-20 mg daily for long-term management of acid reflux disease
  • 40 mg on the preceding evening for prophylaxis of acid aspiration
  • IV Injection recommended for various conditions

Administration

  • Direction for use of IV Injection for intravenous administration
  • Direction for use of IV Infusion as an intravenous infusion over a period of 20-30 minutes or more
  • Reconstituted solution should not be mixed with other drugs

Interaction

  • Decreased intragastric acidity affecting the absorption of ketoconazole
  • Metabolism through cytochrome P450 enzyme system affecting elimination of diazepam, phenytoin, and warfarin
  • Monitoring and dose adjustment recommended with warfarin or phenytoin
  • Increased plasma concentrations of Gem and clarithromycin during concomitant administration
  • No evidence of interaction with phenacetin, theophylline, caffeine, etc.

Contraindications

  • Patients with known hypersensitivity to Omeprazole
  • Suspicion of gastric ulcer with possibility of malignancy

Side Effects

  • Mild and reversible adverse reactions
  • Skin rash, urticaria, and pruritus reported
  • Diarrhoea, headache, constipation, nausea/vomiting, flatulence, and abdominal pain reported
  • Paraesthesia, dizziness, light headedness, and feeling faint associated with treatment
  • Mental confusion, agitation, depression, and hallucinations reported predominantly in severely ill patients
  • Arthritic and myalgic symptoms reported
  • Increases in liver enzymes observed

Pregnancy & Lactation

  • Omeprazole indicated to be used during pregnancy
  • No information available on passage into breast milk or effects on neonate
  • Breast-feeding should be discontinued if essential use of Omeprazole is considered

Precautions & Warnings

  • Avoid concomitant use of clopidogrel and Gem
  • PPI therapy associated with increased risk for osteoporosis-related fractures
  • Atrophic gastritis noted occasionally in long-term treatment
  • Concomitant use with methotrexate may lead to toxicities
