Indications
- Gastric and duodenal ulcer
- NSAID-associated duodenal and gastric ulcer
- Prophylaxis in patients with a history of NSAID-associated duodenal and gastric ulcer
- Gastro-esophageal reflux disease
- Long-term management of acid reflux disease
- Acid-related dyspepsia
- Severe ulcerating reflux esophagitis
- Prophylaxis of acid aspiration during general anesthesia
- Zollinger-Ellison syndrome
- Helicobacter pylori-induced peptic ulcer
Pharmacology
- Inhibitor of gastric acid secretion
- Blocks hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system
- Onset of the antisecretory effect occurs within one hour
- Maximum effect occurs within two hours
- Inhibition of secretion lasts up to 72 hours
- Secretory activity returns gradually over 3 to 5 days after discontinuation
Dosage
- 20 mg once daily for gastric and duodenal ulcer for 4-8 weeks
- 10-20 mg daily for long-term management of acid reflux disease
- 10-20 mg once daily for acid-related dyspepsia for 2-4 weeks
- 40 mg on the preceding evening, then 40 mg 2-6 hours before surgery for prophylaxis of acid aspiration
- Recommended doses and regimen for Zollinger-Ellison syndrome and Helicobacter pylori eradication regimen
- Pediatric use: 10-20 mg once daily for children weighing 10-20 kg, 20-40 mg once daily for children weighing over 20 kg
Administration
- IV Injection: Given slowly over a period of 5 minutes as prophylaxis of acid aspiration
- IV Infusion: Given over a period of 20-30 minutes or more
- Directions for reconstitution and usage of IV injection and infusion
Interaction
- Decreased absorption of ketoconazole during OME treatment
- Metabolism through cytochrome P450 affecting the elimination of diazepam, phenytoin, and warfarin
- Monitoring recommended for patients receiving warfarin or phenytoin
- Plasma concentrations of OME and clarithromycin are increased during concomitant administration
Contraindications
- Known hypersensitivity to omeprazole
- Must exclude possibility of malignancy before treatment in cases of suspected gastric ulcer
Side Effects
- Mild and reversible adverse reactions reported
- Skin rash, urticaria, pruritus, photosensitivity
- Diarrhea, headache, constipation, nausea/vomiting, abdominal pain
- Dry mouth, stomatitis, candidiasis, paraesthesia
- Dizziness, light-headedness, somnolence, insomnia
- Mental confusion, agitation, depression, hallucinations
- Arthritic and myalgic symptoms
- Blurred vision, taste disturbance, peripheral edema
- Increases in liver enzymes
Pregnancy & Lactation
- No adverse effects observed during pregnancy
- No information available on passage into breast milk or effects on neonate
- Discontinue breastfeeding if essential use of omeprazole
Precautions & Warnings
- Avoid concomitant use of clopidogrel and OME
- Increased risk for osteoporosis-related fractures with PPI therapy
- Atrophic gastritis noted in gastric corpus biopsies from long-term OME usage
- Concomitant use with methotrexate may lead to methotrexate toxicities
