Indications

  • Treatment of gastric and duodenal ulcer
  • NSAID-associated duodenal and gastric ulcer treatment
  • Prophylaxis in patients with a history of NSAID-associated ulcers

  • Long-term management of acid reflux disease
  • Treatment of acid-related dyspepsia

  • Prophylaxis of acid aspiration during general anesthesia
  • Treatment of Zollinger-Ellison syndrome
  • Helicobacter pylori-induced peptic ulcer treatment

Pharmacology

  • Inhibitor of gastric acid secretion
  • Blocks hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system
  • Onset of antisecretory effect occurs within one hour
  • Maximum effect occurs within two hours
  • Inhibition of secretion lasts up to 72 hours

Dosage

  • 20 mg once daily for benign gastric and duodenal ulcer
  • 40 mg daily for severe or recurrent cases
  • 10-20 mg daily for prevention of relapse in duodenal ulcer
  • 20 mg once daily for NSAID-associated duodenal or gastric ulcer
  • 20 mg once daily for 4 weeks for gastro-esophageal reflux disease
  • 40 mg once daily for refractory gastro-esophageal reflux disease

Administration

  • Omeprazole IV injection for acid aspiration prophylaxis
  • Omeprazole IV infusion over a period of 20-30 minutes
  • Omeprazole IV infusion dissolved in saline or 5% Dextrose for infusion

Interaction

  • Decreased intragastric acidity affecting ketoconazole absorption
  • Metabolized through cytochrome P450 affecting elimination of diazepam, phenytoin, and warfarin
  • Plasma concentrations increased during concomitant administration with clarithromycin

Contraindications

  • Known hypersensitivity to Omeprazole
  • Suspicion of gastric ulcer malignancy

Side Effects

  • Skin rash, urticaria, pruritus
  • Photosensitivity, bullous eruption, erythema multiforme
  • Diarrhea, headache, constipation
  • Nausea/vomiting, flatulence, abdominal pain

Pregnancy & Lactation

  • No adverse effects on pregnancy or health of fetus/newborn child
  • No information available on passage into breast milk or its effects on neonate

Precautions & Warnings

  • Avoid concomitant use of clopidogrel and Omeprazole
  • Increased risk for osteoporosis-related fractures
  • Atrophic gastritis noted occasionally in long-term treatment
