Indications

  • Treatment of gastric and duodenal ulcer
  • NSAID-associated duodenal and gastric ulcer treatment
  • Prophylaxis in patients with a history of NSAID-associated duodenal and gastric ulcer
  • Gastro-esophageal reflux disease treatment
  • Long-term management of acid reflux disease
  • Acid-related dyspepsia treatment
  • Severe ulcerating reflux esophagitis treatment
  • Prophylaxis of acid aspiration during general anesthesia
  • Zollinger-Ellison syndrome treatment
  • Helicobacter pylori-induced peptic ulcer treatment

Pharmacology

  • Inhibitor of gastric acid secretion
  • Blocks hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system
  • Onset of antisecretory effect within one hour
  • Maximum effect within two hours
  • Inhibition of secretion lasts up to 72 hours
  • Gradual return of secretory activity over 3 to 5 days after discontinuation

Dosage

  • 20 mg once daily for benign gastric and duodenal ulcer (4-8 weeks)
  • 10-20 mg daily for long-term management of acid reflux disease
  • 40 mg on the preceding evening, then 40 mg 2-6 hours before surgery for prophylaxis of acid aspiration
  • Omeprazole regimen in different disease conditions

Administration

  • Omeprazole IV injection for intravenous administration only
  • Direction for use of IV Injection and IV Infusion
  • Reconstitution guidelines for IV injection and infusion
  • Use of freshly prepared solution

Interaction

  • Decreased intragastric acidity affecting ketoconazole absorption
  • Metabolism through cytochrome P450, potentially delaying elimination of certain drugs
  • Monitoring and potential dose adjustment for patients receiving warfarin or phenytoin
  • Plasma concentration changes during concomitant administration
  • No evidence of interaction with certain drugs

Contraindications

  • Known hypersensitivity to omeprazole
  • Exclusion of malignancy before treatment in suspected gastric ulcer cases

Side Effects

  • Mild and reversible adverse reactions
  • Skin rash, urticaria, and pruritus
  • Photosensitivity, bullous eruption, and erythema multiforme
  • Diarrhoea, headache, constipation, nausea/vomiting, flatulence, and abdominal pain
  • Dry mouth, stomatitis, and candidiasis
  • Paraesthesia, dizziness, light headedness, and feeling faint
  • Somnolence, insomnia, and vertigo
  • Arthritic and myalgic symptoms
  • Reversible mental confusion, agitation, depression, and hallucinations
  • Increased liver enzymes

Pregnancy & Lactation

  • No adverse effects on pregnancy or the health of the fetus/newborn child based on epidemiological studies
  • Use during pregnancy, with breastfeeding discontinuation recommendation
  • Safety and effectiveness not established in pediatric patients under 18 years of age

Precautions & Warnings

  • Avoid concomitant use of clopidogrel and omeprazole
  • Potential association of PPI therapy with increased risk for osteoporosis-related fractures
  • Atrophic gastritis noted in long-term treated patients
  • Potential methotrexate toxicities with PPIs
