Indications

  • Gastric and duodenal ulcer
  • NSAID-associated duodenal and gastric ulcer
  • Prophylaxis in patients with a history of NSAID-associated duodenal and gastric ulcer
  • Gastro-esophageal reflux disease
  • Long-term management of acid reflux disease
  • Acid-related dyspepsia
  • Severe ulcerating reflux esophagitis
  • Prophylaxis of acid aspiration during general anesthesia
  • Zollinger-Ellison syndrome
  • Helicobacter pylori-induced peptic ulcer

Pharmacology

  • Inhibitor of gastric acid secretion
  • Blocks hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system
  • Onset of the antisecretory effect occurs within one hour
  • Maximum effect occurs within two hours
  • Inhibition of secretion lasts up to 72 hours

Administration

  • Direction for use of IV Injection
  • Direction for use of IV Infusion
  • Intravenous administration only
  • IV infusion over a period of 20-30 minutes or more
  • Solution for IV injection is obtained by adding 10 ml water for injection to the vial containing powder
  • IV infusion should be given as an intravenous infusion over a period of 20-30 minutes or more

Interaction

  • Decreased intragastric acidity affecting the absorption of ketoconazole
  • Metabolism through cytochrome P450 affecting the elimination of diazepam, phenytoin, and warfarin
  • Monitoring and potential dose adjustment for patients receiving warfarin or phenytoin
  • Increased plasma concentrations during concomitant administration with clarithromycin
  • No evidence of interaction with phenacetin, theophylline, caffeine, propranolol, metoprolol, cyclosporin, lidocaine, quinidine, estradiol, amoxycillin, or antacids
  • No effect on absorption by alcohol or food
  • Interaction with digoxin leading to a 10% increase in bioavailability

Contraindications

  • Known hypersensitivity to Omeprazole
  • Need for exclusion of malignancy when gastric ulcer is suspected

Side Effects

  • Skin rash, urticaria, and pruritus
  • Photosensitivity, bullous eruption, erythema multiforme, angioedema, and alopecia
  • Diarrhoea and headache
  • Gastrointestinal reactions including constipation, nausea/vomiting, flatulence, and abdominal pain
  • Dry mouth, stomatitis, and candidiasis
  • Paraesthesia
  • Dizziness, light-headedness, and feeling faint
  • Somnolence, insomnia, and vertigo

Pregnancy & Lactation

  • Results from prospective epidemiological studies indicating no adverse effects on pregnancy or fetal health
  • Use during pregnancy deemed safe
  • No information available on passage into breast milk or effects on the neonate
  • Discontinuation of breastfeeding if essential use of Omeprazole is considered

Precautions & Warnings

  • Avoid concomitant use of clopidogrel
  • Potential association with increased risk for osteoporosis-related fractures
  • Atrophic gastritis noted occasionally in gastric corpus biopsies from long-term treatment
  • Concomitant use with methotrexate may lead to methotrexate toxicities
