Indications
- Treatment of gastric and duodenal ulcer

Pharmacology
- Inhibitor of gastric acid secretion
- Blocks hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system
- Onset of antisecretory effect within one hour
- Maximum effect within two hours
- Inhibition of secretion lasts up to 72 hours
- Gradual return of secretory activity over 3 to 5 days after discontinuation

Interaction
- Decreased intragastric acidity affecting absorption of ketoconazole
- Metabolism through cytochrome P450 affecting elimination of diazepam, phenytoin, and warfarin
- Monitoring and potential dose reduction for warfarin or phenytoin when co-administered
- Increased plasma concentrations during concomitant administration with clarithromycin
- No evidence of interaction with phenacetin, theophylline, caffeine, propranolol, metoprolol, cyclosporin, lidocaine, quinidine, estradiol, amoxycillin, or antacids
Contraindications
- Known hypersensitivity to omeprazole
- Need to exclude possibility of malignancy before treatment for suspected gastric ulcer
Side Effects
- Mild and reversible adverse reactions
- Reported skin rash, urticaria, pruritus, photosensitivity, bullous eruption, and erythema multiforme
- Reported gastrointestinal reactions like diarrhea, nausea/vomiting, flatulence, and abdominal pain
- Reported neurological and psychotropic effects
Pregnancy & Lactation
- No adverse effects on pregnancy or fetus/newborn child observed in prospective epidemiological studies
- Use during pregnancy considered safe
- No information available on passage into breast milk or effects on the neonate
- Breastfeeding should be discontinued if essential use of omeprazole
Precautions & Warnings
- Avoid concomitant use with clopidogrel
- Possible association with increased risk for osteoporosis-related fractures
- Atrophic gastritis noted in patients treated long-term
- Potential methotrexate toxicities with concomitant use of PPIs
