Indications

  • Treatment of Gastric and duodenal ulcer
  • Treatment of NSAID-associated duodenal and gastric ulcer
  • Prophylaxis in patients with a history of NSAID-associated duodenal and gastric ulcer
  • Treatment of Gastro-esophageal reflux disease
  • Long-term management of acid reflux disease
  • Treatment of Acid-related dyspepsia
  • Treatment of Severe ulcerating reflux esophagitis
  • Prophylaxis of acid aspiration during general anesthesia
  • Treatment of Zollinger-Ellison syndrome
  • Helicobacter pylori-induced peptic ulcer

Pharmacology

  • Inhibitor of gastric acid secretion
  • Blocks hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system in the gastric parietal cell
  • Onset of antisecretory effect occurs within one hour
  • Maximum effect occurs within two hours
  • Inhibition of secretion lasts up to 72 hours
  • Secretory activity returns gradually over 3 to 5 days after discontinuation

Dosage

  • Oral administration for various conditions and durations
  • IV Injection for prophylaxis of acid aspiration
  • IV Injection for duodenal ulcer, gastric ulcer, or reflux oesophagitis

Administration

  • Directions for use of IV Injection

  • Intravenous injection should be given slowly over 2 to 5 minutes
  • Intravenous infusion over a period of 20-30 minutes or more

Interaction

  • Decreased intragastric acidity affecting absorption of ketoconazole
  • Metabolism via cytochrome P450 affecting elimination of diazepam, phenytoin, and warfarin
  • Monitoring of patients receiving warfarin or phenytoin recommended
  • Plasma concentrations increased during concomitant administration with clarithromycin
  • No change in blood concentration of phenytoin during continuous treatment with phenytoin
  • No change in coagulation time during continuous treatment with warfarin

Contraindications

  • Known hypersensitivity to omeprazole
  • Exclusion of malignancy suspicion before treatment

Side Effects

  • Mild and reversible adverse reactions reported
  • Skin rash, urticaria, and pruritus reported
  • Photosensitivity, bullous eruption, erythema multiforme, angioedema, alopecia reported
  • Diarrhoea and headache reported
  • Gastrointestinal reactions including constipation, nausea/vomiting, flatulence, abdominal pain reported

Pregnancy & Lactation

  • No adverse effects of Omeprazole on pregnancy or fetus/newborn child in epidemiological studies
  • Omeprazole can be used during pregnancy
  • No information available on passage into breast milk or effects on the neonate
  • Breast-feeding should be discontinued if essential use of Omeprazole

Precautions & Warnings

  • Avoid concomitant use with clopidogrel
  • Increased risk for osteoporosis-related fractures observed with PPI therapy
  • Atrophic gastritis noted occasionally in long-term treatment
  • Concomitant use with methotrexate may lead to methotrexate toxicities
