Indications

Pharmacology
- Proton pump inhibitor
- Suppresses gastric acid secretion
- Specific inhibition of the H+/K+-ATPase in the gastric parietal cell
- Better acid control than racemic proton pump inhibitors
- Enteric-coated pellet formulation
- Peak plasma levels occur at approximately 1.5 hours
- Distribution is 97% bound to plasma proteins
- Extensively metabolized in the liver
- Plasma elimination half-life is approximately 1-1.5 hours
Dosage
- Oral dosage form
- Healing of Erosive Esophagitis: 20 mg or 40 mg Once Daily for 4-8 Weeks
- Maintenance of Healing of Erosive Esophagitis: 20 mg Once Daily
- Symptomatic GERD: 20 mg Once Daily for 4 Weeks
- Helicobacter Pylori eradication: Triple Therapy for 10 days
- Zollinger-Ellison Syndrome: 20-80 mg Once Daily
- Acid-related Dyspepsia: 20-40 mg Once Daily for 2-4 weeks
- Duodenal ulcer: 20 mg Once Daily for 2-4 weeks
- IV Injection or IV infusion for various conditions
Administration
- Tablet or capsule should be swallowed whole and taken one hour before a meal
- Oral Suspension should be taken with water or administered through a tube
- IV Injection should be prepared with 0.9% Sodium Chloride and administered intravenously over a period of at least 3 minutes
Interaction
- Extensively metabolized by CYP2C19 and CYP3A4
- No clinically relevant interactions with certain CYP enzymes
- May interfere with absorption of certain drugs where gastric pH is important
- Increased plasma levels observed with certain co-administered drugs
Contraindications
- Known hypersensitivity to any of the formulation

Pregnancy & Lactation
- No adequate and well-controlled studies in pregnant women
- No teratogenic effects observed in animal studies

- Symptomatic response to therapy does not preclude the presence of gastric malignancy
- Information for patients regarding capsule administration and use with antacids
- Use in paediatric patients, geriatric patients, hepatic insufficiency, and renal insufficiency
Use in Special Populations
- Safety and effectiveness not established in paediatric patients
- No overall differences in safety and efficacy between elderly and younger individuals
- No dosage adjustment recommended for mild to moderate hepatic insufficiency
- Pharmacokinetics not expected to be altered in patients with renal impairment
