Indications

Pharmacology
- Mode of action: Proton pump inhibitor suppressing gastric acid secretion
- Absorption: Peak plasma levels occur at approximately 1.5 hours after oral administration
- Distribution: 97% bound to plasma proteins with constant plasma protein binding over a specific concentration range
- Metabolism: Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system
- Excretion: Plasma elimination half-life is approximately 1–1.5 hours with less than 1% of parent drug excreted in the urine
Dosage
- 20 mg or 40 mg Once Daily for 4-8 Weeks for healing of erosive esophagitis
- 20 mg Once Daily for maintenance of healing of erosive esophagitis
- 20 mg Once Daily for symptomatic GERD
- Triple Therapy for Helicobacter Pylori eradication
- 20-80 mg Once Daily for Zollinger-Ellison Syndrome
- 20-40 mg Once Daily for Acid-related Dyspepsia
- 20 mg Once Daily for 2-4 weeks for Duodenal ulcer, 20-40 mg Once Daily for 4-8 weeks for Gastric ulcer
Administration
- Tablet or capsule: Swallowed whole and taken one hour before a meal
- Oral Suspension: Administered with specific instructions for mixing and consumption
- IV Injection: Prepared and administered intravenously over a specific period
Interaction
Extensively metabolized in the liver by CYP2C19 and CYP3A4, with potential interactions with several drugs
Contraindications
Contraindicated in patients with known hypersensitivity to any of the formulation
Side Effects
Common adverse events include headache, diarrhea, nausea, flatulence, abdominal pain, constipation, and dry mouth
Pregnancy & Lactation
- Pregnancy: No adequate and well-controlled studies in pregnant women, with animal studies showing no teratogenic effects
- Lactation: Excretion of esomeprazole in milk has not been studied, and breastfeeding should be discontinued if the use of esomeprazole is considered essential
Precautions & Warnings
- General: Symptomatic response to therapy does not preclude the presence of gastric malignancy

Use in Special Populations

- Hepatic Insufficiency: No dosage adjustment recommended for patients with mild to moderate hepatic insufficiency
- Renal Insufficiency: No alteration expected in the pharmacokinetics of esomeprazole in patients with renal impairment
Overdose Effects
Signs of acute toxicity include reduced motor activity, changes in respiratory frequency, tremor, ataxia, and intermittent clonic convulsions, with no specific antidote known
