Indications
- Relief from chronic heartburn symptoms

Pharmacology
- Proton pump inhibitor
- Suppresses gastric acid secretion
- Enteric-coated pellet formulation
- Metabolized in the liver by CYP2C19 and CYP3A4
- Peak plasma levels occur at approximately 1.5 hours
- Plasma elimination half-life of approximately 1–1.5 hours
Dosage
- Oral:
- Healing of erosive esophagitis: 20 mg or 40 mg once daily for 4-8 weeks
- Maintenance of healing of erosive esophagitis: 20 mg once daily
- Symptomatic GERD: 20 mg once daily for 4 weeks
- Helicobacter pylori eradication: 40 mg once daily for 10 days
- Zollinger-Ellison Syndrome: 20-80 mg once daily
- Acid-related dyspepsia: 20-40 mg once daily for 2-4 weeks
- Duodenal ulcer: 20 mg once daily for 2-4 weeks
- Gastric ulcer: 20-40 mg once daily for 4-8 weeks
- IV Injection or Infusion:
- GERD with erosive esophagitis: 20 mg or 40 mg once daily for adult patients
- Pediatric patients (1 year to 17 years) - 10 mg to 20 mg once daily based on body weight
- Risk reduction of rebleeding of Gastric or Duodenal Ulcers following therapeutic endoscopy in adults: 80 mg followed by continuous infusion
Administration
- Tablet or capsule should be swallowed whole and taken one hour before a meal
- Oral suspension should be taken with water, and any remaining medicine should be consumed immediately
- IV injection should be administered over a period of at least 3 minutes
Interaction
- Extensively metabolized in the liver by CYP2C19 and CYP3A4
- Inhibition of gastric acid secretion
- Potential interference with the absorption of certain drugs
Contraindications
- Known hypersensitivity to any of the formulation

Pregnancy & Lactation
- No adequate and well-controlled studies in pregnant women

Precautions & Warnings
- Symptomatic response to therapy with esomeprazole does not preclude the presence of gastric malignancy
- Patients should be informed about proper administration, including swallowing whole tablets or capsules
Use in Special Populations
- Pediatric Use: Safety and effectiveness not established
- Geriatric Use: No overall differences in safety and efficacy observed
- Hepatic Insufficiency: No dosage adjustment recommended for patients with mild to moderate hepatic insufficiency
- Renal Insufficiency: Pharmacokinetics not expected to be altered relative to healthy volunteers
Overdose Effects
- Common signs of acute toxicity include reduced motor activity, respiratory changes, tremor, and convulsions
