Indications

  • Relief from chronic heartburn symptoms
  • Healing of erosive esophagitis
  • Maintenance of healing of erosive esophagitis
  • Eradication of Helicobacter pylori infection in patients with duodenal ulcer disease
  • Treatment of Zollinger-Ellison Syndrome
  • Management of acid-related Dyspepsia
  • Healing of duodenal and gastric ulcers

Pharmacology

  • Proton pump inhibitor
  • Suppression of gastric acid secretion
  • Specific inhibition of the H+/K+-ATPase in the gastric parietal cell
  • First single optical isomer of proton pump inhibitor
  • Enteric-coated pellet formulation of esomeprazole magnesium
  • Metabolized in the liver by the cytochrome P450 (CYP) enzyme system

Dosage

  • Oral dosage form
  • IV Injection or IV infusion for specific conditions
  • Dosage range for healing, maintenance, symptomatic relief, eradication, and syndromes

Administration

  • Swallowed whole and taken one hour before a meal (tablet or capsule)

Interaction

  • Metabolism by CYP2C19 and CYP3A4
  • No clinically significant interactions with phenytoin, warfarin, quinidine, clarithromycin or amoxicillin

Contraindications

  • Known hypersensitivity to any of the formulation

  • Symptomatic response does not preclude the presence of gastric malignancy
  • Specific administration guidelines for patients with difficulty swallowing capsules
  • Possibility of using antacids while taking esomeprazole

Use in Special Populations

  • Safety and effectiveness in paediatric patients have not been established
  • No overall differences in safety and efficacy between the elderly and younger individuals
  • No dosage adjustment recommended for patients with mild to moderate hepatic insufficiency
  • No alteration of Pharmacokinetics in patients with renal impairment

Overdose Effects

  • Lethal dose observed in animal studies
  • Major signs of acute toxicity include reduced motor activity, changes in respiratory frequency, tremor, and convulsions
  • No specific antidote known
