Indications
- Relief from chronic heartburn symptoms and other symptoms associated with GERD
- Healing of erosive esophagitis
- Maintenance of healing of erosive esophagitis
- Eradication of Helicobacter pylori infection in patients with duodenal ulcer disease
- Treating Zollinger-Ellison Syndrome
- Treatment of acid related Dyspepsia
- Healing of duodenal and gastric ulcers
Pharmacology
- Proton pump inhibitor
- Suppression of gastric acid secretion
- Inhibition of H+/K+-ATPase in gastric parietal cell
- Absorption: Enteric-coated pellet formulation
- Peak plasma levels occur at approximately 1.5 hours after oral administration
- Extensive metabolism in the liver by the cytochrome P450 enzyme system
- Excretion: Less than 1% of parent drug excreted in the urine
Dosage
- Oral dosage form: 20 mg or 40 mg Once Daily for 4-8 Weeks for healing of erosive esophagitis
- Triple Therapy for Helicobacter pylori eradication: Esomeprazole 40 mg Once Daily for 10 days, Amoxicillin 1000 mg Twice Daily for 10 days, Clarithromycin 500 mg Twice Daily for 10 days
- IV Injection or IV infusion for GERD with Erosive Esophagitis
- Different dosages for Zollinger-Ellison Syndrome, acid-related Dyspepsia, duodenal ulcer, and gastric ulcer
Administration
- Tablet or capsule should be swallowed whole and taken one hour before a meal
- Oral Suspension: Should be mixed with water and taken immediately
- IV Injection: Administered intravenously over a period of at least 3 minutes
Interaction
- Metabolized in the liver by CYP2C19 and CYP3A4
- No clinically relevant interactions with specific CYP enzymes
- Potential interference with CYP2C19
- Inhibition of gastric acid secretion may interfere with the absorption of certain drugs
Contraindications
- Known hypersensitivity to any of the formulation

Pregnancy & Lactation
- No adequate and well-controlled studies in pregnant women
- Animal studies revealed no teratogenic effects
- Excretion in milk not studied
Precautions & Warnings
- Symptomatic response does not preclude the presence of gastric malignancy
- Guidelines for patients who have difficulty swallowing capsules
- Use of antacids while taking Esotid
Use in Special Populations
- Safety and effectiveness in pediatric patients not established
- No overall differences in safety and efficacy observed between the elderly and younger individuals
- No dosage adjustment recommended for patients with mild to moderate hepatic insufficiency
Overdose Effects
- Lethal to rats at high doses
- Reduced motor activity
- Tremor
- Ataxia
- Intermittent clonic convulsions
- No specific antidote
