Indications

  • Treatment of gastric and duodenal ulcer
  • NSAID-associated duodenal and gastric ulcer
  • Prophylaxis in patients with a history of NSAID-associated duodenal and gastric ulcer
  • Gastro-esophageal reflux disease
  • Long-term management of acid reflux disease
  • Acid-related dyspepsia
  • Severe ulcerating reflux esophagitis
  • Prophylaxis of acid aspiration during general anesthesia
  • Zollinger-Ellison syndrome
  • Helicobacter pylori-induced peptic ulcer

Pharmacology

  • Omeprazole, a substituted benzimidazole
  • Inhibitor of gastric acid secretion
  • Blocks hydrogen-potassium-adenosine triphosphatase
  • Onset of antisecretory effect within one hour
  • Maximum effect occurring within two hours
  • Inhibition of secretion lasts up to 72 hours
  • Secretory activity returns gradually over 3 to 5 days

Administration

  • Direction for IV Injection administration
  • Direction for IV Infusion administration
  • Slow intravenous injection for IV Injection
  • Intravenous infusion over a period of 20-30 minutes or more
  • Reconstituted solution should not be mixed with any other drug

Interaction

  • Decreased intragastric acidity affects ketoconazole absorption
  • Metabolized in the liver through cytochrome P450
  • Delayed elimination of diazepam, phenytoin, and warfarin
  • Monitoring for patients receiving warfarin or phenytoin
  • Increased plasma concentrations during concomitant administration with clarithromycin
  • No evidence of an interaction with various drugs

Contraindications

  • Known hypersensitivity to Omeprazole
  • Gastric ulcer suspected with possibility of malignancy

Side Effects

  • Skin rash, urticaria, pruritus
  • Photosensitivity, bullous eruption, erythema multiforme
  • Diarrhea, headache, constipation, nausea/vomiting
  • Dizziness, light-headedness, somnolence, insomnia
  • Reversible mental confusion, agitation, depression, hallucinations
  • Arthritic and myalgic symptoms, blurred vision, taste disturbance

Pregnancy & Lactation

  • No adverse effects on pregnancy or fetus/newborn child
  • Can be used during pregnancy
  • No information available on passage into breast milk
  • Discontinue breastfeeding if Omeprazole use is considered essential
  • Safety and effectiveness not established in pediatric patients less than 18 years of age

Precautions & Warnings

  • Avoid concomitant use of clopidogrel and Omeprazole
  • Increased risk for osteoporosis-related fractures
  • Atrophic gastritis noted occasionally in long-term use
  • Concomitant use with methotrexate may lead to toxicities
