Recommended dosage

  • Adults: 10-20 mg every 6-8 hours daily
  • Children: 2-4 ml suspension/10 kg body weight every 6-8 hours daily

Maximum daily dose

80 mg

Indicated for

  • Dyspeptic symptom complex
  • Acute nausea and vomiting
  • Parkinson's disease-induced nausea and vomiting
  • Radiological studies for speeding barium transit

Pharmacological action

  • Dopamine antagonist
  • Blocks dopamine receptors in the Chemoreceptor Trigger Zone (CTZ) and stomach
  • Restores normal motility and tone of the upper gastrointestinal tract
  • Facilitates gastric emptying
  • Enhances antral and duodenal peristalsis
  • Regulates contraction of the pylorus
  • Increases esophageal peristalsis and lower esophageal sphincter pressure

Interaction with other drugs

  • Anticholinergic drugs: Antagonizes the antidyspeptic effect
  • Metabolic pathway: Primarily through CYP3A4
  • Inhibitors of CYP3A4:
    • Azole antifungals
    • Macrolide antibiotics
    • HIV protease inhibitors
    • Nefazodone
  • Does not interfere with:
    • Neuroleptics
    • Dopaminergic agonists

Contraindications

  • Hypersensitivity to the drug
  • Neonates
  • Gastrointestinal hemorrhage
  • Mechanical obstruction or perforation
  • Prolactin releasing pituitary tumor (prolactinoma)

Side effects

  • Transient intestinal cramps
  • Extrapyramidal phenomena (rare)
  • Increase in plasma prolactin level leading to neuroendocrinological phenomena
  • Rare allergic reactions like rash and urticaria

Pregnancy and lactation

  • Pregnancy: Should only be used during the first trimester if justified by the anticipated therapeutic benefit
  • Lactation: Not recommended unless potential benefits outweigh risks
