Description
- A dopamine antagonist medication used to treat nausea and vomiting.
Indications

Pharmacology
- Dopamine antagonist
- Blocks dopamine receptors in the Chemoreceptor Trigger Zone (CTZ) and stomach
- Gastroprokinetic action
- Restores normal motility and tone of the upper gastrointestinal tract
- Enhances antral and duodenal peristalsis
- Regulates contraction of the pylorus
Dosage & Administration
- Usual Dose (Adults): 10-20 mg every 6-8 hours daily
- Maximum Dose (Adults): 80 mg daily
- Dose for Children: 2-4 ml suspension/10 kg body weight every 6-8 hours daily
- By Rectum (Adults): 30-60 mg every 4-8 hours
- Maximum Period of Treatment: 12 weeks
Interaction
- Anticholinergic drugs may antagonize the effect of domperidone
- Antacids and antisecretory drugs lower oral bioavailability
- Concomitant use with CYP3A4 inhibitors may increase plasma levels
- May influence absorption of concomitantly administered drugs
Contraindications
- Known hypersensitivity to the drug
- Neonates
- Gastrointestinal hemorrhage, mechanical obstruction, or perforation
- Prolactin releasing pituitary tumor (prolactinoma)
Side Effects
- Transient intestinal cramps
- Extrapyramidal phenomena (rare)
- Increased plasma prolactin level
- Rare allergic reactions such as rash and urticaria
Pregnancy & Lactation
- Use during the first trimester justified by anticipated therapeutic benefit
- No evidence of increased risk of malformations in humans
- Excreted in breast milk, caution advised for nursing mothers
Precautions & Warnings
- Use with caution in children due to risk of extra-pyramidal reactions
- Use with caution in patients with hepatic impairment
Use in Special Populations
- Use in infants with great caution and under close medical supervision
- Use with caution in patients with hepatic impairment
- Use with caution in patients with severe renal insufficiency
