Indications
- Treatment of benign prostatic hyperplasia (BPH)
- Treatment of lower urinary tract symptoms (LUTS)
- Relief from urinary frequency
- Relief from nocturia
- Relief from incomplete emptying
- Relief from urinary hesitancy
Pharmacology
- Selectively antagonizes post-synaptic a1 adrenoreceptors
- Relaxes tone of prostate smooth muscle
- Relaxes prostate capsule
- Relaxes bladder neck
- Relaxes proximal urethra
- Preferentially binds to prostatic a1 receptors
- Reduces resting urethral pressure
- Inhibits urethral hypertonia-induced sympathetic nervous stimulation
Dosage
- Recommended dose of 10mg once daily for BPH after a meal
- Recommended dose of 10mg daily for acute urinary retention in patients 65 years and older
Administration
- Tablet should be swallowed whole
Interaction
- Contraindicated with alpha-1-receptor blockers
- To be taken into account with antihypertensive drugs
- To be taken into account with nitrates
- To be taken into account with potent CYP3A4 inhibitors
- Effect increased with repeated dosing of ketoconazole
- Potential interaction with general anesthetics
- No observed interaction with warfarin
- No observed interaction with digoxin
- No observed interaction with hydrochlorothiazide
- No observed interaction with atenolol
Contraindications
- Caution with patients receiving antihypertensive medications or nitrates
- Caution in patients with acute cardiac failure
- Caution in patients with hypersensitivity to alpha-1-blockers
- Careful administration in patients being treated with antihypertensives
- Patient warning for possible postural hypotension
- Caution in patients with history of acquired QTc prolongation
- Caution in patients taking drugs known to increase the QTc interval
- Not to be administered to patients with severe renal impairment
- Caution in patients with congenital QTc prolongation
- Caution in coronary patients
- Caution in patients with known or past use of alpha-1-blockers for cataract surgery
- Contains hydrogenated castor oil which may cause stomach upset and diarrhea
Side Effects
- Common: faintness/dizziness, headache
- Uncommon: syncope, vertigo, malaise, drowsiness
- Uncommon: vision abnormal
- Uncommon: tachycardia, palpitations, hypotension (postural)
- Very rare: New onset, aggravation or recurrence of angina pectoris
- Uncommon: flushing
- Not known: intraoperative floppy iris syndrome
- Not known: atrial fibrillation
- Not known: neutropenia, thrombocytopenia
- Uncommon: rhinitis
- Common: nausea, abdominal pain
- Uncommon: diarrhea, dry mouth, vomiting
- Frequency unknown: hepatocellular injury, cholestatic liver disease
- Uncommon: rash, pruritus
- Very rare: urticaria, angioedema
- Frequency unknown: priapism
- Common: asthenia
- Uncommon: flushes, edema, chest pain
Precautions & Warnings
- Tablets to be withdrawn 24 hours before surgery if general anesthetics are to be administered
- Stop taking if symptoms of angina pectoris start or worsen
Use in Special Populations
- Not demonstrated efficacy in children aged 2 to 16 years
Overdose Effects
- Hospitalization required
- Supine position recommended
- Conventional treatment of hypotension necessary
- Consider vasoconstrictor for significant hypotension
