Pharmacology

  • Inhibitor of gastric acid secretion
  • Blocks hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system
  • Onset of antisecretory effect within one hour
  • Maximum effect occurring within two hours
  • Inhibition of secretion lasts up to 72 hours
  • Gradual return of secretory activity over 3 to 5 days after discontinuation

Dosage

  • 20 mg once daily for benign gastric and duodenal ulcer
  • 10-20 mg daily for long-term management of acid reflux disease
  • 40 mg on the preceding evening for prophylaxis of acid aspiration
  • 20 mg twice daily for Helicobacter pylori eradication regimen in peptic ulcer
  • 10-20 mg once daily for severe ulcerating reflux esophagitis in pediatric use

Administration

  • Must be given as a slow intravenous injection
  • IV infusion should be given over a period of 20-30 minutes or more

Interaction

  • Decreased intragastric acidity affecting the absorption of ketoconazole
  • Metabolism in the liver through cytochrome P450 delaying the elimination of diazepam, phenytoin, and warfarin
  • Plasma concentrations increased during concomitant administration with clarithromycin
  • No evidence of interaction with phenacetin, theophylline, caffeine, propranolol, metoprolol, cyclosporin, lidocaine, quinidine, estradiol, amoxicillin, antacids, piroxicam, diclofenac, naproxen, and alcohol or food
  • Increase in the bioavailability of digoxin during simultaneous treatment

Contraindications

  • Known hypersensitivity to omeprazole
  • Suspicion of gastric ulcer without excluding the possibility of malignancy

Side Effects

  • Mild and reversible adverse reactions
  • Reports of skin rash, urticaria, pruritus, photosensitivity, bullous eruption, and erythema multiforme
  • Reports of diarrhea, headache, constipation, nausea/vomiting, flatulence, and abdominal pain
  • Isolated cases of dry mouth, stomatitis, candidiasis, paraesthesia, dizziness, light headedness, feeling faint, somnolence, insomnia, vertigo, mental confusion, agitation, depression, hallucinations, arthritic and myalgic symptoms, blurred vision, taste disturbance, and peripheral edema
  • Reports of increased liver enzymes

Pregnancy & Lactation

  • No adverse effects indicated on pregnancy or on the health of the fetus/newborn child
  • Can be used during pregnancy

Precautions & Warnings

  • Avoid concomitant use with clopidogrel
  • Potential association with an increased risk for osteoporosis-related fractures of the hip, wrist, or spine
  • Occasional atrophic gastritis in gastric corpus biopsies from patients treated long-term
  • Potential for methotrexate toxicities with concomitant use
