Zatral 10 mg (Tablet (Extended Release))

Indications

• Treatment of benign prostatic hyperplasia (BPH)
• Management of lower urinary tract symptoms (LUTS)
• Relief of urinary frequency and nocturia
• Improvement of incomplete emptying and urinary hesitancy

Pharmacology

• Selective antagonist of post-synaptic a1 adrenoreceptors
• Relaxation of prostate smooth muscle
• Competitive and selective binding to post synaptic a1-adrenergic receptors
• Reduced resting urethral pressure
• Inhibition of urethral hypertonia-induced sympathetic nervous stimulation

Dosage

• Recommended 10 mg dose once daily after a meal for BPH
• 10 mg daily for acute urinary retention (AUR) in patients 65 years and older
• Treatment duration of 3-4 days for AUR

Administration

• Tablet should be swallowed whole

Interaction

• Contraindicated with alpha-1-receptor blockers
• Considerations with antihypertensive drugs and nitrates
• Potential interaction with potent CYP3A4 inhibitors like ketoconazole and ritonavir
• Increased exposure when administered with ketoconazole under fed conditions

Contraindications

• Caution in patients receiving antihypertensive medications or nitrates
• Use with caution in patients with acute cardiac failure
• Gradual initiation in patients with hypersensitivity to alpha-1-blockers
• Monitor blood pressure regularly especially at the beginning of treatment
• Careful administration in patients with known or acquired QTc prolongation

Side Effects

• Common: faintness/dizziness, headache
• Uncommon: syncope, vertigo, malaise, drowsiness
• Not known: intraoperative floppy iris syndrome
• Uncommon: tachycardia, palpitations
• Very rare: New onset, aggravation or recurrence of angina pectoris in patients with pre-existing coronary artery disease

Pregnancy & Lactation

• Not applicable due to the type of indication

Precautions & Warnings

• Discontinue use if symptoms of angina pectoris worsen
• Discontinue tablets 24 hours before surgery if general anesthetics are to be administered

Use in Special Populations

• Not indicated for use in the paediatric population

Overdose Effects

• Hospitalization and conventional treatment for hypotension in case of overdosage
• Consideration of vasoconstrictor for significant hypotension
