Indications

Pharmacology
- Binds to penicillin binding proteins
- Inhibits peptidoglycan synthesis
- Broad-spectrum bactericidal
- Effective against gram-negative organisms
- Activity against gram-positive bacteria
Dosage & Administration
- Recommended adult dosage: 1 gm every 12 hourly
- Variation of dosage based on severity of infection
- Increased dosage for infections caused by Pseudomonas spp.
- Usual children dosage range: 100-150 mg/kg/day
- Increased children dosage for severe infections
- Recommended neonates dosage: 50 mg/kg/day
- Increased neonates dosage for severe infections
- 500 mg dosage for gonorrhoea treatment
Interaction
- Increased nephrotoxicity with aminoglycoside antibiotics
Contraindications
- Hypersensitivity to cefotaxime or cephalosporin antibiotics
Side Effects
- Relatively infrequent and mild adverse reactions
- Adverse effects include candidiasis, rashes, fever
- Transient rises in liver transaminase and/or alkaline phosphatase
- Diarrhoea
- Rare occurrence of pseudomembranous colitis
- Changes in renal function observed with high doses
- Hypersensitivity reactions reported
Pregnancy & Lactation
- Safety not established in human pregnancy
- Caution advised for administration during first trimester
- Excretion in milk
Precautions & Warnings
- Caution in individuals with gastrointestinal disease
- Dosage reduction in renal insufficiency
- No clinical evidence supporting dosage change in severe renal dysfunction
Use in Special Populations
- Dosage reduction in severe renal failure
- Further dosage adjustment based on infection course and patient's condition
