Description

  • Cefotaxime is a broad spectrum bactericidal 3rd generation parenteral cephalosporin antibiotic.

Indications

  • Treatment of septicaemia
  • Treatment of respiratory tract infections
  • Treatment of urinary tract infections
  • Treatment of soft-tissue infections
  • Treatment of bone and joint infections
  • Prophylactic treatment for certain postoperative infections

Pharmacology

  • Binds to penicillin binding proteins
  • Inhibits final transpeptidation step of peptidoglycan synthesis
  • Arrests cell wall assembly resulting in bacterial cell death
  • Broad spectrum bactericidal
  • Active against gram-negative organisms
  • Active against gram-positive bacteria

Dosage and administration

  • Recommended dosage for adults: 1 gm every 12 hours
  • Dosage variation based on severity of infection and patient's condition
  • Usual dosage range for children: 100-150 mg/kg/day in 2 to 4 divided doses
  • Recommended dosage for neonates: 50 mg/kg/day in 2 to 4 divided doses
  • Single dose of 500 mg for gonorrhoea

Interaction

  • Nephrotoxicity with concomitant administration of cephalosporins and aminoglycoside antibiotics

Contraindications

  • Hypersensitivity to cefotaxime or cephalosporin group of antibiotics

Pregnancy and lactation

Precautions and warnings

  • Prescribed with caution in individuals with a history of gastrointestinal disease
  • Reduced dosage for patients with renal insufficiency
  • No clinical evidence supporting necessity of changing dosage in patients with profound renal dysfunction

Use in special populations

  • Dosage reduction in severe renal failure
