Description
- Cefotaxime is a broad spectrum bactericidal 3rd generation parenteral cephalosporin antibiotic.
Indications
- Treatment of septicaemia
- Treatment of respiratory tract infections
- Treatment of urinary tract infections
- Treatment of soft-tissue infections
- Treatment of bone and joint infections
- Prophylactic treatment for certain postoperative infections
Pharmacology
- Binds to penicillin binding proteins
- Inhibits final transpeptidation step of peptidoglycan synthesis
- Arrests cell wall assembly resulting in bacterial cell death
- Broad spectrum bactericidal
- Active against gram-negative organisms
- Active against gram-positive bacteria
Dosage and administration
- Recommended dosage for adults: 1 gm every 12 hours
- Dosage variation based on severity of infection and patient's condition
- Usual dosage range for children: 100-150 mg/kg/day in 2 to 4 divided doses
- Recommended dosage for neonates: 50 mg/kg/day in 2 to 4 divided doses
- Single dose of 500 mg for gonorrhoea
Interaction
- Nephrotoxicity with concomitant administration of cephalosporins and aminoglycoside antibiotics
Contraindications
- Hypersensitivity to cefotaxime or cephalosporin group of antibiotics
Side effects
- Candidiasis
- Skin rashes
- Fever
- Transient rises in liver transaminase
- Diarrhoea
- Pseudomembranous colitis
- Changes in renal function
- Hypersensitivity reactions
Pregnancy and lactation
- Not established for human pregnancy
- Excreted in milk
Precautions and warnings
- Prescribed with caution in individuals with a history of gastrointestinal disease
- Reduced dosage for patients with renal insufficiency
- No clinical evidence supporting necessity of changing dosage in patients with profound renal dysfunction
Use in special populations
- Dosage reduction in severe renal failure
