Mode of Action

  • Binds to penicillin binding proteins, inhibits peptidoglycan synthesis, broad spectrum bactericidal effect

Dosage for Adults

  • 1 gm every 12 hours for mild to moderate infections
  • Up to 12 gm daily in severe infections

Dosage for Children

  • 100-150 mg/kg/day in 2 to 4 divided doses
  • Up to 200 mg/kg/day in very severe infections

Dosage for Neonates

  • 50 mg/kg/day in 2 to 4 divided doses
  • 150-200 mg/kg/day in severe infections

Dosage for Gonorrhoea

  • 500 mg as a single dose

Interaction

  • Increased nephrotoxicity with aminoglycoside antibiotics

Contraindications

  • Hypersensitivity to cefotaxime or the cephalosporin group of antibiotics

  • Candidiasis
  • Rashes
  • Fever
  • Transient rises in liver transaminase and/or alkaline phosphatase
  • Diarrhoea
  • Pseudomembranous colitis

Pregnancy & Lactation

  • Not established for safety in human pregnancy
  • Excreted in the milk

Precautions

  • Prescribed with caution in individuals with a history of gastrointestinal disease
  • Dosage reduction in patients with transient or persistent reduction of urinary output due to renal insufficiency

Special Population Uses

  • Dosage reduction in severe renal failure
  • Further dosage adjustment according to the course of infection
