Pharmacology

  • Binds to penicillin binding proteins
  • Inhibits final transpeptidation step of peptidoglycan synthesis
  • Broad spectrum bactericidal
  • Active against gram-negative organisms
  • Activity against gram-positive bacteria

Dosage and Administration

  • Recommended dosage for mild to moderate infections in adults: 1 gm every 12 hours
  • Dosage variation according to severity of infection
  • Children dosage range: 100-150 mg/kg/day in 2 to 4 divided doses
  • Neonates recommended dosage: 50 mg/kg/day in 2 to 4 divided doses
  • Single dose of 500 mg for gonorrhoea

Interaction

  • Increased nephrotoxicity with concomitant administration of cephalosporins and aminoglycoside antibiotics

Contraindications

  • Hypersensitivity to cefotaxime or cephalosporins

Side Effects

  • Infrequent and mild adverse reactions
  • Effects include candidiasis, rashes, fever, transient rises in liver enzymes, diarrhoea
  • Pseudomembranous colitis may rarely occur
  • Rare changes in renal function with high doses
  • Hypersensitivity reactions reported

Pregnancy and Lactation

  • Safety not established in human pregnancy
  • Excretion in milk

Precautions and Warnings

  • Prescribed with caution in individuals with history of gastrointestinal disease
  • Dosage adjustment for patients with renal insufficiency
  • No evidence supporting necessity of changing the dosage in patients with renal dysfunction

Use in Special Populations

  • Dosage adjustment in severe renal failure
  • Renal impairment dosage adjustment
  • Further dosage adjustment according to infection course and patient's general condition
