Pharmacology
- Binds to penicillin binding proteins
- Inhibits final transpeptidation step of peptidoglycan synthesis
- Broad spectrum bactericidal
- Active against gram-negative organisms
- Activity against gram-positive bacteria
Dosage and Administration
- Recommended dosage for mild to moderate infections in adults: 1 gm every 12 hours
- Dosage variation according to severity of infection
- Children dosage range: 100-150 mg/kg/day in 2 to 4 divided doses
- Neonates recommended dosage: 50 mg/kg/day in 2 to 4 divided doses
- Single dose of 500 mg for gonorrhoea
Interaction
- Increased nephrotoxicity with concomitant administration of cephalosporins and aminoglycoside antibiotics
Contraindications
- Hypersensitivity to cefotaxime or cephalosporins
Side Effects
- Infrequent and mild adverse reactions
- Effects include candidiasis, rashes, fever, transient rises in liver enzymes, diarrhoea
- Pseudomembranous colitis may rarely occur
- Rare changes in renal function with high doses
- Hypersensitivity reactions reported
Pregnancy and Lactation
- Safety not established in human pregnancy
- Excretion in milk
Precautions and Warnings
- Prescribed with caution in individuals with history of gastrointestinal disease
- Dosage adjustment for patients with renal insufficiency
- No evidence supporting necessity of changing the dosage in patients with renal dysfunction
Use in Special Populations
- Dosage adjustment in severe renal failure
- Renal impairment dosage adjustment
- Further dosage adjustment according to infection course and patient's general condition

- Store below 25°C, protected from light and moisture
- Use reconstituted solution immediately
