Indications
- Treatment of various infections including respiratory tract infections, urinary tract infections, soft-tissue infections, bone and joint infections, obstetric and gynaecological infections, and other bacterial infections
- Prophylactic use to reduce post-operative infections in certain surgical procedures
Pharmacology
- Binds to penicillin binding proteins to inhibit cell wall synthesis in bacteria, leading to cell death
- Broad spectrum bactericidal 3rd generation cephalosporin antibiotic
Dosage & Administration
- Recommended dosage for adults in mild to moderate infections is 1 gm every 12 hours
- Dosage may be varied based on severity of infection and patient condition
- Different dosage ranges for children and neonates
- Specific dosage for gonorrhoea treatment
Interaction
- Increased nephrotoxicity reported with concomitant use of cephalosporins and aminoglycoside antibiotics
Contraindications
- Hypersensitivity to cefotaxime or cephalosporin antibiotics
Side Effects
- Mild and transient adverse reactions include candidiasis, rashes, fever, liver enzyme level changes, and diarrhea
- Rare occurrences of pseudomembranous colitis, renal function changes, encephalopathy, and hypersensitivity reactions
Pregnancy & Lactation
- Safety in human pregnancy not established, cautious use during pregnancy especially in the first trimester
- Excreted in breast milk
Precautions & Warnings
- Caution in patients with gastrointestinal disease
- Dosage adjustment necessary in patients with renal insufficiency
- No evidence to change dosage in patients with renal dysfunction
Use in Special Populations
- Reduced dosage required in severe renal failure
- Individual dosing adjustment based on infection course and patient condition
