Pharmacology

  • Binds to penicillin binding proteins (PBPs)
  • Inhibits transpeptidation step of peptidoglycan synthesis
  • Bacterial cell wall inhibition
  • Broad spectrum bactericidal action
  • Active against gram-negative organisms
  • Activity against gram-positive bacteria

Dosage & Administration

  • Recommended dosage for adults: 1 gm every 12 hours
  • Children dosage range: 100-150 mg/kg/day
  • Neonates dosage: 50 mg/kg/day
  • Dosage in gonorrhoea: 500 mg as a single dose

Interaction

  • Increased nephrotoxicity with concomitant administration of cephalosporins and aminoglycoside antibiotics

Contraindications

  • Hypersensitivity to cefotaxime or cephalosporin antibiotics

  • No clinical evidence supporting dosage change in profound renal dysfunction

Use in Special Populations

  • Dosage reduction in severe renal failure
  • Further dosage adjustment according to infection and patient condition
