Indications

  • Septicaemia
  • Respiratory Tract Infections
  • Urinary Tract Infections
  • Soft-tissue Infection
  • Bone and Joint Infections
  • Obstetric and gynaecological infections
  • Gonorrhoea
  • Meningitis
  • Prophylaxis

Pharmacology

  • Binds to penicillin binding proteins (PBPs)
  • Inhibits peptidoglycan synthesis
  • Broad spectrum bactericidal
  • Active against gram-negative organisms
  • Activity against gram-positive bacteria

Dosage & Administration

  • Recommended dosage for adults: 1 gm every 12 hourly for mild to moderate infections
  • Recommended dosage for children: 100-150 mg/kg/day
  • Recommended dosage for neonates: 50 mg/kg/day
  • Dosage for gonorrhoea: 500 mg as a single dose

Interaction

  • Increased nephrotoxicity with aminoglycoside antibiotics

Contraindications

  • Hypersensitivity to cefotaxime or the cephalosporin group of antibiotics

Side Effects

  • Mild and transient
  • Adverse effects include candidiasis, rashes, fever, transient rises in liver transaminase, and/or alkaline phosphatase, and diarrhea
  • Pseudomembranous colitis may rarely occur
  • Changes in renal function observed with high doses

Pregnancy & Lactation

  • Safety not established in human pregnancy
  • Excreted in the milk

Precautions & Warnings

  • Prescribed with caution in individuals with a history of gastrointestinal disease
  • Total daily dosage should be reduced in patients with renal insufficiency
  • Continued dosage determined by degree of renal impairment

Use in Special Populations

  • Dosage reduction in severe renal failure
  • Dosage may require adjustment according to the course of infection and general condition of the patient
