Indications

  • Essential hypertension
  • Angina pectoris and hypertension co-existing diseases
  • Post myocardial infarction
  • Refractory angina pectoris

Pharmacology

  • Fixed-dose combination of Amlodipine and Atenolol
  • Amlodipine: dihydropyridine calcium antagonist
  • Amlodipine: peripheral vasodilator
  • Amlodipine: reduces peripheral vascular resistance
  • Amlodipine: reduces coronary vasoreactivity
  • Atenolol: cardioselective beta-blocker
  • Atenolol: reduces blood pressure by lowering cardiac output
  • Atenolol: decreases plasma renin activity
  • Atenolol: reduction in myocardial oxygen demand

Dosage & Administration

  • Recommended dosage: Amlodipine and Atenolol 5/25 mg tablet once daily
  • Dosage may be increased as advised by physician
  • Dosage should be individualized

Interaction

  • Disopyramide: Atenolol reduces clearance by 20%
  • Disopyramide: additive negative inotropic effects
  • Ampicillin: reduces Atenolol levels at doses of 1 gm and above
  • Oral antidiabetics and insulin: Beta-blockers may decrease tissue sensitivity to insulin and inhibit insulin secretion

Contraindications

  • Hypersensitivity to either component
  • Sinus bradycardia
  • Second and higher degrees of heart block
  • Cardiogenic shock
  • Hypotension
  • Congestive heart failure
  • Poor left ventricular function

Pregnancy & Lactation

  • Use during pregnancy if expected benefit outweighs potential fetal risk

  • Bronchospasm
  • Renal impairment
  • Hepatic impairment
  • Drug withdrawal: Gradual discontinuation and observation

Overdose Effects

  • Hypotension
  • Congestive cardiac failure
  • Unabsorbed drugs removal by gastric lavage or activated charcoal
  • Symptomatic treatment
