Pharmacology
- Inhibitor of gastric acid secretion
- Blocks hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system in the gastric parietal cell
- Onset of antisecretory effect occurs within one hour
- Maximum effect occurs within two hours
- Inhibition of secretion lasts up to 72 hours
- Gradual return of secretory activity over 3 to 5 days after discontinuation

Contraindications
- Known hypersensitivity

Side Effects

- Reports of skin rash, urticaria, pruritus, photosensitivity, and bullous eruption
- Gastrointestinal reactions including diarrhea, headache, constipation, and nausea/vomiting
- Neurological symptoms such as paraesthesia, dizziness, and somnolence
- Psychological effects including reversible mental confusion, agitation, and depression
- Reports of arthritic and myalgic symptoms, blurred vision, and taste disturbance
- Isolated cases of thrombocytopenia, agranulocytosis, and hepatotoxicity
Pregnancy & Lactation
- No adverse effects on pregnancy or newborn child reported in epidemiological studies
- Use during pregnancy deemed safe based on available information
- No information on passage into breast milk or effects on neonate
- Caution and possible discontinuation of breast-feeding if essential
Precautions & Warnings
- Avoid concomitant use with clopidogrel
- Observational studies suggesting increased risk for osteoporosis-related fractures
- Noted occurrence of atrophic gastritis in patients treated long-term
- Potential methotrexate toxicities with concomitant use
