Indications

  • Prophylaxis in patients with a history of NSAID-associated duodenal and gastric ulcer

  • Long-term management of acid reflux disease
  • Management of acid-related dyspepsia
  • Severe ulcerating reflux esophagitis treatment
  • Prophylaxis of acid aspiration during general anesthesia
  • Zollinger-Ellison syndrome treatment
  • Helicobacter pylori-induced peptic ulcer treatment

Pharmacology

  • Inhibitor of gastric acid secretion
  • Blocks hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system
  • Onset of the antisecretory effect within one hour
  • Maximum effect occurring within two hours
  • Inhibition of secretion lasting up to 72 hours
  • Gradual return of secretory activity over 3 to 5 days

Administration

  • Direction for use of IV Injection

Interaction

  • Decreased intragastric acidity affecting absorption of ketoconazole
  • Metabolism in the liver through cytochrome P450 affecting elimination of diazepam, phenytoin, and warfarin
  • Monitoring of patients receiving warfarin or phenytoin
  • Plasma concentrations of Omeprol and clarithromycin during concomitant administration
  • Phenytoin blood concentration during continuous treatment
  • Coagulation time in patients on continuous treatment with warfarin
  • No evidence of interaction with various medications and antacids

Contraindications

  • Known hypersensitivity to Omeprazole
  • Suspicion of malignancy in gastric ulcer

Side Effects

  • Mild and reversible adverse reactions
  • Skin rash, urticaria, and pruritus
  • Photosensitivity, bullous eruption, and erythema multiforme
  • Gastrointestinal reactions including constipation, nausea/vomiting, and abdominal pain
  • Diarrhoea and headache
  • Dizziness, light-headedness, and feeling faint
  • Reversible mental confusion, agitation, depression, and hallucinations
  • Arthritic and myalgic symptoms
  • Increases in liver enzymes

Pregnancy & Lactation

  • Results from prospective epidemiological studies
  • No adverse effects on pregnancy or on the health of the fetus/newborn child
  • No information available on the passage of Omeprazole into breast milk or its effects on the neonate

Precautions & Warnings

  • Avoid concomitant use of clopidogrel and Omeprazole
  • Increased risk for osteoporosis-related fractures
  • Occasional atrophic gastritis in gastric corpus biopsies from long-term treatment
  • Concomitant use of PPIs with methotrexate may lead to methotrexate toxicities
