Pharmacology

  • Substituted benzimidazole
  • Inhibitor of gastric acid secretion
  • Blocks hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system
  • Onset of the antisecretory effect occurs within one hour
  • Maximum effect occurs within two hours
  • Inhibition of secretion lasts up to 72 hours
  • Secretory activity returns gradually over 3 to 5 days when the drug is discontinued

Dosage

  • Oral administration
  • Benign gastric and duodenal ulcer: 20 mg once daily for 4 weeks in duodenal ulceration, 8 weeks in gastric ulceration
  • NSAID-associated duodenal or gastric ulcer: 20 mg once daily for 4 weeks, continued for further 4 weeks if not fully healed
  • Gastro-esophageal reflux disease: 20 mg once daily for 4 weeks, continued for further 4-8 weeks if not fully healed
  • Long-term management of acid reflux disease: 10-20 mg daily
  • Prophylaxis of acid aspiration: 40 mg on the preceding evening, then 40 mg 2-6 hours before surgery
  • Zollinger-Ellison syndrome: Initially 60 mg once daily; usual range 20-120 mg daily
  • IV Injection for various indications

Administration

  • Direction for use of IV Injection

  • Intravenous administration only
  • IV injection should be given as a slow intravenous injection
  • IV infusion should be given over a period of 20-30 minutes or more

Interaction

  • Decreased intragastric acidity may reduce absorption of ketoconazole during OPmax treatment
  • OPmax is metabolized in the liver through cytochrome P450
  • Potential interactions with diazepam, phenytoin, warfarin, ketoconazole, clarithromycin, digoxin, methotrexate

Contraindications

  • Known hypersensitivity to omeprazole
  • Suspicion of gastric ulcer should exclude the possibility of malignancy before treatment is instituted

Side Effects

  • Mild and reversible adverse reactions
  • Skin rash, urticaria, and pruritus
  • Photosensitivity, bullous eruption, erythema multiforme, angioedema, and alopecia
  • Diarrhea, headache
  • Constipation, nausea/vomiting, flatulence, abdominal pain
  • Dry mouth, stomatitis, candidiasis
  • Paraesthesia
  • Dizziness, light-headedness, feeling faint
  • Somnolence, insomnia, vertigo
  • Reversible mental confusion, agitation, depression, hallucinations
  • Arthritic and myalgic symptoms
  • Blurred vision, taste disturbance, peripheral edema, increased sweating
  • Hepatic and hematological effects

Pregnancy & Lactation

  • No adverse effects reported on pregnancy or fetus/newborn child
  • Use during pregnancy is considered safe
  • No information available on the passage of omeprazole into breast milk
  • Breastfeeding should be discontinued if omeprazole use is considered essential
  • Safety and effectiveness not established in pediatric patients less than 18 years of age

Precautions & Warnings

  • Avoid concomitant use of clopidogrel and OPmax
  • Possible association with increased risk for osteoporosis-related fractures
  • Atrophic gastritis noted occasionally in long-term use
  • Concomitant use with methotrexate may lead to methotrexate toxicities
