Indications

  • Gastric and duodenal ulcer
  • NSAID-associated duodenal and gastric ulcer
  • Prophylaxis in patients with a history of NSAID-associated duodenal and gastric ulcer
  • Gastro-esophageal reflux disease
  • Long-term management of acid reflux disease
  • Acid-related dyspepsia
  • Severe ulcerating reflux esophagitis
  • Prophylaxis of acid aspiration during general anesthesia
  • Zollinger-Ellison syndrome
  • Helicobacter pylori-induced peptic ulcer

Pharmacology

  • Inhibitor of gastric acid secretion
  • Blocks hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system
  • Onset of the antisecretory effect occurs within one hour
  • Maximum effect occurs within two hours
  • Inhibition of secretion lasts up to 72 hours
  • Return of secretory activity takes 3 to 5 days

Dosage

  • 20 mg once daily for benign gastric and duodenal ulcer (4-8 weeks duration)
  • 20 mg once daily for NSAID-associated duodenal or gastric ulcer (4-8 weeks duration)
  • 20 mg once daily for gastro-esophageal reflux disease (4-8 weeks duration; maintenance dose of 20 mg)
  • 10-20 mg daily for long-term management of acid reflux disease
  • 10-20 mg once daily for acid-related dyspepsia (2-4 weeks duration)
  • 40 mg on the preceding evening, then 40 mg 2-6 hours before surgery for prophylaxis of acid aspiration
  • 0-120 mg daily for Zollinger-Ellison syndrome (divided doses if daily dose >80 mg)
  • 20 mg twice daily for Helicobacter pylori eradication regimen in peptic ulcer disease, in combination with antimicrobial agents
  • 10-20 mg once daily for 4-12 weeks in severe ulcerating reflux esophagitis (paediatric use)

Administration

  • Omeprazole IV injection should be given as a slow intravenous injection
  • Omeprazole IV infusion should be given over a period of 20-30 minutes or more
  • Reconstituted solution should not be mixed or co-administered with any other drug in the same infusion set

Interaction

  • Reduced absorption of ketoconazole
  • Delayed elimination of diazepam, phenytoin, and warfarin
  • Increased plasma concentrations of Proceptin and clarithromycin during concomitant administration
  • No evidence of interaction with phenacetin, theophylline, caffeine, propranolol, metoprolol, cyclosporin, lidocaine, quinidine, estradiol, amoxicillin, or antacids
  • No effect of alcohol or food on Omeprazole absorption
  • Increased bioavailability of digoxin during simultaneous treatment with Proceptin

Contraindications

  • Known hypersensitivity to Omeprazole
  • Suspicion of gastric ulcer without excluding the possibility of malignancy

Side Effects

  • Skin rash, urticaria, and pruritus
  • Photosensitivity, bullous eruption, erythema multiforme, angioedema, and alopecia
  • Diarrhoea, headache, constipation, nausea/vomiting, flatulence, and abdominal pain
  • Dry mouth, stomatitis, candidiasis, and paraesthesia
  • Dizziness, light-headedness, feeling faint, somnolence, insomnia, and vertigo
  • Reversible mental confusion, agitation, depression, and hallucinations
  • Arthritic and myalgic symptoms
  • Blurred vision, taste disturbance, peripheral oedema, increased sweating, gynaecomastia, leucopenia, thrombocytopenia, agranulocytosis, pancytopenia, anaphylactic shock, malaise, fever, bronchospasm, encephalopathy, hepatitis with or without jaundice, interstitial nephritis, hepatic failure, and increases in liver enzymes

Pregnancy & Lactation

  • No adverse effects on pregnancy and fetus/newborn child reported
  • Use during pregnancy is considered safe
  • No information available on passage of Omeprazole into breast milk or effects on the neonate
  • Breastfeeding should be discontinued if essential

Precautions & Warnings

  • Avoid concomitant use of clopidogrel and Proceptin
  • Possible association of PPI therapy with increased risk for osteoporosis-related fractures
  • Atrophic gastritis observed in patients treated long-term with Proceptin
  • Potential methotrexate toxicities with concomitant use of PPIs
