Indications

  • Gastric and duodenal ulcer
  • NSAID-associated duodenal and gastric ulcer
  • Prophylaxis in patients with a history of NSAID-associated duodenal and gastric ulcer
  • Gastro-esophageal reflux disease
  • Long-term management of acid reflux disease
  • Acid-related dyspepsia
  • Severe ulcerating reflux esophagitis
  • Prophylaxis of acid aspiration during general anesthesia
  • Zollinger-Ellison syndrome
  • Helicobacter pylori-induced peptic ulcer

Pharmacology

  • Inhibitor of gastric acid secretion
  • Blocks hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system
  • Onset of antisecretory effect occurs within one hour
  • Maximum effect occurs within two hours
  • Inhibition of secretion lasts up to 72 hours
  • Gradual return of secretory activity over 3 to 5 days after discontinuation

Dosage

  • Oral dosage for various conditions
  • IV injection dosage for prophylaxis of acid aspiration, duodenal ulcer, gastric ulcer, reflux esophagitis, Zollinger-Ellison syndrome

Interaction

  • Can reduce absorption of ketoconazole
  • Metabolized in the liver through cytochrome P450
  • Can delay the elimination of diazepam, phenytoin, and warfarin
  • Plasma concentrations increased during concomitant administration with clarithromycin
  • No evidence of interaction with phenacetin, theophylline, caffeine, propranolol, metoprolol, cyclosporin, lidocaine, quinidine, estradiol, amoxycillin, antacids, piroxicam, diclofenac, naproxen
  • Increased bioavailability of digoxin with simultaneous treatment

Contraindications

  • Known hypersensitivity to omeprazole
  • Suspicion of gastric ulcer should exclude the possibility of malignancy before treatment

Side Effects

  • Skin rash, urticaria, pruritus
  • Photosensitivity, bullous eruption, erythema multiforme, angioedema, alopecia
  • Diarrhea, headache
  • Constipation, nausea/vomiting, flatulence, abdominal pain
  • Dry mouth, stomatitis, candidiasis
  • Paraesthesia
  • Dizziness, light headedness, feeling faint
  • Somnolence, insomnia, vertigo
  • Reversible mental confusion, agitation, depression, hallucinations
  • Arthritic and myalgic symptoms
  • Blurred vision, taste disturbance, peripheral edema, increased sweating, gynaecomastia
  • Leucopenia, thrombocytopenia, agranulocytosis, pancytopenia
  • Anaphylactic shock, malaise, fever, bronchospasm
  • Encephalopathy in patients with pre-existing severe liver disease
  • Hepatitis with or without jaundice, interstitial nephritis, hepatic failure
  • Increases in liver enzymes

Pregnancy & Lactation

  • Results from epidemiological studies indicate no adverse effects on pregnancy or on the health of the fetus/newborn child
  • Omeprazole can be used during pregnancy
  • No information available on the passage of omeprazole into breast milk or its effects on the neonate
  • Discontinue breastfeeding if essential use of omeprazole is considered

Precautions & Warnings

  • Avoid concomitant use with clopidogrel
  • Observational studies suggest an increased risk for osteoporosis-related fractures with PPI therapy
  • Atrophic gastritis noted occasionally in patients treated long-term with omeprazole
  • Concomitant use with methotrexate may lead to methotrexate toxicities
