Indications
- To relieve chronic heartburn symptoms and other symptoms associated with GERD
- For the healing of erosive esophagitis
- For the maintenance of healing of erosive esophagitis
- In combination with amoxicillin and clarithromycin for the eradication of Helicobacter pylori infection in patients with duodenal ulcer disease
- For Zollinger-Ellison Syndrome
- For acid-related Dyspepsia
- For duodenal and gastric ulcers
Pharmacology
- Proton pump inhibitor
- Suppresses gastric acid secretion
- Specific inhibition of the H+/K+-ATPase in the gastric parietal cell
- First single optical isomer of proton pump inhibitor
- Better acid control than racemic proton pump inhibitors
- Peak plasma levels occur at approximately 1.5 hours after oral administration
- Oral bioavailability is approximately 90%
- Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system
- Plasma elimination half-life is approximately 1-1.5 hours
Dosage
- Oral dosage form for various conditions
- IV Injection or IV infusion for GERD and risk reduction of rebleeding of gastric or duodenal ulcers
- Different doses for different age groups and conditions
Administration
- Tablet or capsule should be swallowed whole and taken one hour before a meal
- Oral suspension should be taken and administered with specific instructions
- IV Injection requires specific preparation and administration guidelines
Interaction
- Extensively metabolized in the liver by CYP2C19 and CYP3A4
- In vitro and in vivo studies show no clinically relevant interactions with specific CYP enzymes
- Potential interference with the absorption of drugs where gastric pH is important for bioavailability
- Increased plasma levels of certain drugs with co-administration
Contraindications
- Contraindicated in patients with known hypersensitivity to any of the formulation

Pregnancy & Lactation

Precautions & Warnings
- Symptomatic response does not rule out the presence of gastric malignancy
- Specific instructions for patients who have difficulty swallowing capsules
- Use of antacids while taking the medication
Use in Special Populations
- Safety and effectiveness not established in pediatric patients
- No overall differences in safety and efficacy observed in the elderly
- No dosage adjustment recommended for patients with mild to moderate hepatic insufficiency
Overdose Effects
- Lethal dose in rats reported at a significantly higher level than the human dose
- Signs of acute toxicity include reduced motor activity, changes in respiratory frequency, tremor, and convulsions
- No specific antidote known, treatment should be symptomatic and supportive

- Store at a temperature not exceeding 30°C in a dry place
