Indications

  • Relief from chronic heartburn symptoms
  • Healing of erosive esophagitis
  • Maintenance of healing of erosive esophagitis
  • Eradication of Helicobacter pylori infection in patients with duodenal ulcer disease
  • Treatment for Zollinger-Ellison Syndrome
  • Treatment for acid-related dyspepsia
  • Treatment for duodenal and gastric ulcer

Pharmacology

  • Mechanism of action: Proton pump inhibitor that suppresses gastric acid secretion
  • Absorption: Peak plasma levels occur at approximately 1.5 hours after oral administration
  • Distribution: 97% bound to plasma proteins
  • Metabolism: Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system
  • Excretion: Plasma elimination half-life is approximately 1–1.5 hours

Dosage

  • 20 mg or 40 mg Once Daily for 4-8 Weeks for healing of erosive esophagitis
  • 20 mg Once Daily for maintenance of healing of erosive esophagitis
  • 20 mg Once Daily for symptomatic GERD for 4 Weeks
  • Triple Therapy (Esomeprazole 40 mg Once Daily, Amoxicillin 1000 mg Twice Daily, Clarithromycin 500 mg Twice Daily) for 10 days for Helicobacter pylori eradication
  • Dose of 20-80 mg once daily for Zollinger-Ellison Syndrome
  • 20-40 mg once daily for 2-4 weeks for acid-related dyspepsia
  • 20 mg once daily for 2-4 weeks for duodenal ulcer, 20-40 mg once daily for 4-8 weeks for gastric ulcer
  • Dosage for IV Injection or IV infusion varies for different conditions and patient age groups

Administration

  • Tablet or capsule should be swallowed whole and taken one hour before a meal
  • Oral Suspension should be taken with water and consumed within 30 minutes
  • IV Injection must be administrated intravenously over a period of at least 3 minutes

Interactions

  • Metabolized in the liver by CYP2C19 and CYP3A4
  • Not likely to inhibit certain CYP enzymes
  • May interfere with the absorption of drugs where gastric pH is an important determinant of bioavailability

Contraindications

  • Known hypersensitivity to any of the formulation

Pregnancy & Lactation

  • No adequate and well-controlled studies in pregnant women
  • Animal studies have revealed no teratogenic effects
  • Breast-feeding should be discontinued if the use of esomeprazole is considered essential

Precautions & Warnings

  • Symptomatic response does not preclude the presence of gastric malignancy
  • Patients should take capsules at least one hour before meals
  • Antacids may be used while taking Esocon

Use in Special Populations

  • Safety and effectiveness in pediatric patients have not been established
  • No overall differences in safety and efficacy between the elderly and younger individuals
  • No dosage adjustment is recommended for patients with mild to moderate hepatic insufficiency
  • Pharmacokinetics not expected to be altered in patients with renal impairment

Overdose Effects

  • Major signs of acute toxicity are reduced motor activity, changes in respiratory frequency, tremor, ataxia, and intermittent clonic convulsions
  • No specific antidote known
  • Treatment should be symptomatic and supportive

Storage Conditions

  • Store at a temperature not exceeding 30°C in a dry place, protect from light and moisture
