Pharmacology
- Proton pump inhibitor
- Specific inhibition of the H+/K+-ATPase in the gastric parietal cell
- First single optical isomer of proton pump inhibitor
- Enteric-coated pellet formulation of esomeprazole magnesium
- Gastric acid suppression
- Metabolized in the liver by cytochrome P450 (CYP) enzyme system
- Plasma elimination half-life of approximately 1-1.5 hours
- High plasma protein binding (97%)
- Extensive metabolism in the liver
- Minimal excretion in urine
Dosage
- Oral dosage form
- Healing of erosive esophagitis: 20mg or 40mg once daily for 4-8 weeks
- Symptomatic GERD: 20mg once daily for 4 weeks
- Helicobacter pylori eradication: Esomeprazole 40mg once daily for 10 days
- IV Injection: 20mg or 40mg once daily for adult patients
- Pediatric patients dosage based on body weight
- Dosage adjustment for risk reduction of rebleeding of gastric or duodenal ulcers
Administration
- Tablet or capsule should be swallowed whole and taken one hour before a meal
- Oral suspension should be taken with water and within a specific time frame
- IV Injection solution for injection preparation and administration instructions
Interaction
- Extensively metabolized in the liver by CYP2C19 and CYP3A4
- Potential interference with absorption of certain drugs
- No clinically significant interactions with specific drugs
- Inhibition of gastric acid secretion

- Symptomatic response does not preclude the presence of gastric malignancy
- Instructions for patients with difficulty swallowing capsules
- Use of antacids while taking esomeprazole
Use in Special Populations
- Safety and effectiveness not established in pediatric patients
- No overall differences in safety and efficacy observed in the elderly
- No dosage adjustment recommended for patients with mild to moderate hepatic insufficiency
- Pharmacokinetics not expected to be altered in patients with renal impairment
Overdose Effects
- Lethal effects observed in animal studies at high doses
- Symptomatic and supportive treatment recommended in case of overdose
