Indications

  • Relief from chronic heartburn symptoms
  • Treatment of symptoms associated with GERD
  • Healing of erosive esophagitis
  • Maintenance of healing of erosive esophagitis
  • Eradication of Helicobacter pylori infection in patients with duodenal ulcer disease
  • Treatment of Zollinger-Ellison Syndrome
  • Treatment of acid-related Dyspepsia
  • Treatment of duodenal and gastric ulcers

Pharmacology

  • Proton pump inhibitor
  • Suppression of gastric acid secretion
  • Inhibition of H+/K+-ATPase
  • Single optical isomer of proton pump inhibitor
  • Better acid control than racemic proton pump inhibitors
  • Peak plasma levels at approximately 1.5 hours
  • Increased Cmax and AUC with increased dose
  • Extensively metabolized in the liver
  • Dependency on CYP2C19 for metabolism
  • Distribution: 97% bound to plasma proteins
  • Plasma elimination half-life of approximately 1–1.5 hours

Dosage

  • Oral dosage form
  • Healing of Erosive Esophagitis: 20 mg or 40 mg Once Daily for 4-8 Weeks
  • Maintenance of Healing of Erosive Esophagitis: 20 mg Once Daily
  • Symptomatic GERD: 20 mg Once Daily for 4 Weeks
  • Helicobacter Pylori eradication: Triple Therapy for 10 days
  • Zollinger-Ellison Syndrome: 20-80 mg once daily
  • Acid-related Dyspepsia: 20-40 mg once daily for 2-4 weeks
  • Duodenal ulcer: 20 mg once daily for 2-4 weeks
  • Gastric ulcer: 20-40 mg once daily for 4-8 weeks
  • IV Injection: Various dosages for adult and pediatric patients
  • Infusion for risk reduction of rebleeding of Gastric or Duodenal Ulcers in adults

Administration

  • Tablet or capsule should be swallowed whole and taken one hour before a meal
  • Oral Suspension: Instructions for preparation and administration
  • IV Injection: Preparation and administration guidelines

Interaction

  • Metabolized by CYP2C19 and CYP3A4
  • No clinically relevant interactions with certain CYP enzymes
  • Potential for interference with the absorption of drugs affected by gastric pH
  • Co-administration effects with other drugs including clarithromycin and amoxicillin

Contraindications

  • Known hypersensitivity to any of the formulation

Pregnancy & Lactation

  • No adequate and well-controlled studies in pregnant women
  • Animal studies have revealed no teratogenic effects

Precautions & Warnings

  • Symptomatic response to therapy does not preclude the presence of gastric malignancy
  • Specific instructions for patients who have difficulty swallowing capsules
  • Use of antacids while taking the medication

Use in Special Populations

  • Safety and effectiveness in pediatric patients have not been established
  • No overall differences in safety and efficacy observed between the elderly and younger individuals
  • No dosage adjustment recommended for patients with mild to moderate hepatic insufficiency
  • Pharmacokinetics not expected to be altered in patients with renal impairment

Overdose Effects

  • Lethal effects observed in animal studies at high doses
  • No specific antidote known
  • Expected to be extensively protein bound
