Indications

  • Relief from chronic heartburn symptoms
  • Healing of erosive esophagitis
  • Maintenance of healing of erosive esophagitis
  • Eradication of Helicobacter pylori infection in patients with duodenal ulcer disease
  • Treatment of Zollinger-Ellison Syndrome
  • Management of Acid-related Dyspepsia
  • Treatment of Duodenal & Gastric ulcer

Pharmacology

  • Proton pump inhibitor
  • Suppression of gastric acid secretion
  • Inhibition of the H+/K+-ATPase in the gastric parietal cell
  • Enteric-coated pellet formulation of esomeprazole magnesium
  • Absorption peak plasma levels occur at approximately 1.5 hours
  • Systemic bioavailability is approximately 90%
  • Plasma protein binding is constant over the concentration range
  • Metabolized in the liver by the cytochrome P450 (CYP) enzyme system
  • Plasma elimination half-life is approximately 1–1.5 hours

Dosage

  • Oral dosage form for healing of erosive esophagitis, symptomatic GERD, Helicobacter Pylori eradication, Zollinger-Ellison Syndrome, Acid-related Dyspepsia, Duodenal & Gastric ulcer
  • IV Injection or IV infusion for GERD with Erosive Esophagitis, Risk reduction of rebleeding of Gastric or Duodenal Ulcers in adults
  • Dosage adjustment for pediatric patients

Administration

  • Tablet or capsule should be swallowed whole and taken one hour before a meal
  • Oral Suspension should be taken with water and stirred well
  • IV Injection solution for injection is prepared by adding 0.9% Sodium Chloride
  • IV injection must be administered intravenously over a period of at least 3 minutes

Interaction

  • Metabolized in the liver by CYP2C19 and CYP3A4
  • Not likely to inhibit CYPs 1A2, 2A6, 2C9, 2D6, 2E1 and 3A4
  • Potential interference with the absorption of drugs where gastric pH is important
  • Increased plasma levels observed with diazepam

Contraindications

  • Known hypersensitivity to any of the formulation

Pregnancy & Lactation

  • No adequate and well-controlled studies in pregnant women
  • Animal studies have revealed no teratogenic effects

Precautions & Warnings

  • Symptomatic response does not preclude the presence of gastric malignancy
  • Tablet/capsule can be mixed with applesauce for swallowing
  • Antacids may be used while taking

Use in Special Populations

  • Safety and effectiveness in pediatric patients not established
  • No overall differences in safety and efficacy observed in geriatric use
  • No dosage adjustment recommended for patients with mild to moderate hepatic insufficiency
  • Pharmacokinetics not expected to be altered in patients with renal impairment

Overdose Effects

  • Major signs of acute toxicity include reduced motor activity, changes in respiratory frequency, tremors, and clonic convulsions
  • No specific antidote known

  • Store at a temperature not exceeding 30°C in a dry place

  • Keep out of reach of children
