Description
Esomeprazole is a proton pump inhibitor used to relieve chronic heartburn and other symptoms associated with GERD, heal erosive esophagitis, maintain healing of erosive esophagitis, eradicate Helicobacter pylori infection, treat Zollinger-Ellison Syndrome, acid-related dyspepsia, duodenal & gastric ulcers.
Indications
- Relief from chronic heartburn symptoms
- Healing of erosive esophagitis
- Maintenance of healing of erosive esophagitis
- Eradication of Helicobacter pylori infection in patients with duodenal ulcer disease
- Treatment of Zollinger-Ellison Syndrome
- Treatment of acid-related dyspepsia
- Treatment of duodenal & gastric ulcers
Pharmacology
- Proton pump inhibitor
- Suppresses gastric acid secretion
- Better acid control than racemic proton pump inhibitors
- Enteric-coated pellet formulation for peak plasma levels
Absorption
- Peak plasma levels occur at approximately 1.5 hours
- Cmax increases proportionally with the dose
Distribution
- 97% bound to plasma proteins
- Steady-state apparent volume of distribution is approximately 16 L
Metabolism
- Extensively metabolized in the liver by CYP enzymes
- Metabolites lack anti-secretory activity
Excretion
- Plasma elimination half-life of approximately 1-1.5 hours
- Less than 1% excreted in the urine
Dosage
- Oral and IV dosage forms available
- Dosage varies for different indications and patient groups
Administration
- Tablet or capsule should be swallowed whole and taken one hour before a meal
- Oral suspension and IV injection instructions provided
Interactions
- Extensively metabolized by CYP2C19 and CYP3A4
- Potential interactions with diazepam and drugs affected by gastric pH
