Pharmacology

  • Inhibitor of gastric acid secretion
  • Blocks hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system
  • Onset of antisecretory effect within one hour
  • Maximum effect occurring within two hours
  • Inhibition of secretion lasts up to 72 hours
  • Gradual return of secretory activity over 3 to 5 days after discontinuation

Interaction

  • Decreased intragastric acidity affecting ketoconazole absorption
  • Metabolism through cytochrome P450 affecting diazepam, phenytoin, warfarin elimination

Contraindications

  • Known hypersensitivity

Side Effects

  • Mild and reversible adverse reactions
  • Skin rash, urticaria, pruritus, photosensitivity, bullous eruption, erythema multiforme, angioedema, alopecia
  • Diarrhoea, headache, constipation, nausea/vomiting, flatulence, abdominal pain
  • Dry mouth, stomatitis, candidiasis, paraesthesia
  • Dizziness, light headedness, feeling faint, somnolence, insomnia, vertigo
  • Reversible mental confusion, agitation, depression, hallucinations
  • Arthritic and myalgic symptoms
  • Other side effects such as blurred vision, taste disturbance, peripheral oedema, increased sweating, gynaecomastia, leucopenia, thrombocytopenia, agranulocytosis, pancytopenia, anaphylactic shock, malaise, fever, bronchospasm, encephalopathy, hepatitis, interstitial nephritis, hepatic failure, increases in liver enzymes

Pregnancy & Lactation

  • No adverse effects on pregnancy or the health of the fetus/newborn child
  • Use during pregnancy
  • No information available on passage into breast milk or its effects on the neonate
  • Discontinuation of breastfeeding if use is considered essential
  • Safety and effectiveness not established in pediatric patients less than 18 years of age

Precautions & Warnings

  • Avoid concomitant use of clopidogrel
  • Potential increased risk for osteoporosis-related fractures
  • Atrophic gastritis in patients treated long-term
  • Potential methotrexate toxicities with concomitant use of PPIs
