Indications

  • Gastric and duodenal ulcer
  • NSAID-associated duodenal and gastric ulcer
  • Prophylaxis in patients with a history of NSAID-associated duodenal and gastric ulcer
  • Gastro-esophageal reflux disease
  • Long-term management of acid reflux disease
  • Acid-related dyspepsia
  • Severe ulcerating reflux esophagitis
  • Prophylaxis of acid aspiration during general anesthesia
  • Zollinger-Ellison syndrome
  • Helicobacter pylori-induced peptic ulcer

Pharmacology

  • Inhibitor of gastric acid secretion
  • Blocks hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system
  • Onset of antisecretory effect within one hour
  • Maximum effect within two hours
  • Inhibition of secretion lasts up to 72 hours
  • Gradual return of secretory activity over 3 to 5 days after discontinuation

Dosage

  • 20 mg once daily for benign gastric and duodenal ulcer
  • 20 mg once daily for NSAID-associated duodenal or gastric ulcer
  • 20 mg once daily for gastro-esophageal reflux disease
  • 10-20 mg daily for long-term management of acid reflux disease
  • 10-20 mg once daily for acid-related dyspepsia
  • 40 mg on the preceding evening for prophylaxis of acid aspiration
  • 60 mg once daily for Zollinger-Ellison syndrome
  • 20 mg twice daily for Helicobacter pylori eradication regimen
  • 10-20 mg once daily for severe ulcerating reflux esophagitis (Child>1 year)
  • 40 mg as intravenous injection for prophylaxis of acid aspiration
  • 40 mg intravenously for duodenal ulcer, gastric ulcer or reflux esophagitis
  • 60 mg daily for Zollinger-Ellison syndrome (intravenous)

Administration

  • Omeprazole IV injection for intravenous administration only
  • Omeprazole IV injection given as a slow intravenous injection
  • Omeprazole IV infusion given as intravenous infusion over a period of 20-30 minutes or more

Interaction

  • Decreased intragastric acidity affecting absorption of ketoconazole
  • Metabolized in the liver through cytochrome P450
  • Possible delay in elimination of diazepam, phenytoin, and warfarin
  • Increased plasma concentrations during concomitant administration with clarithromycin
  • No evidence of interaction with phenacetin, theophylline, caffeine, and more

Contraindications

  • Known hypersensitivity to omeprazole
  • Suspected gastric ulcer with possibility of malignancy

Side Effects

  • Well tolerated with generally mild and reversible adverse reactions
  • Reported cases of skin rash, urticaria, pruritus, photosensitivity, and more

Pregnancy & Lactation

  • No adverse effects reported on pregnancy or newborn child
  • Use during pregnancy considered safe
  • No information available on passage into breast milk
  • Discontinuation of breast-feeding if essential use is considered

Precautions & Warnings

  • Avoid concomitant use with clopidogrel
  • Possible association with increased risk of osteoporosis-related fractures
  • Atrophic gastritis noted occasionally in long-term treatment
  • Risk of methotrexate toxicities with concomitant use
