Pharmacology
- Inhibitor of gastric acid secretion
- Blocks hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system
- Onset of antisecretory effect occurs within one hour
- Maximum effect occurring within two hours
- Inhibition of secretion lasts up to 72 hours

Interaction
- Decreased intragastric acidity affecting ketoconazole absorption
- Metabolism through cytochrome P450 affecting diazepam, phenytoin, and warfarin elimination
- Increased plasma concentrations during concomitant administration
- No evidence of interaction with phenacetin, theophylline, caffeine, propranolol, metoprolol, cyclosporin, lidocaine, quinidine, estradiol, amoxicillin, or antacids
- No effect on absorption by alcohol or food
- Increased bioavailability of digoxin during simultaneous treatment with Promezol
Contraindications
- Known hypersensitivity
Side Effects
- Mild and reversible adverse reactions
- Skin rash, urticaria, and pruritus
- Photosensitivity, bullous eruption, and erythema multiforme
- Diarrhoea and headache
- Gastrointestinal reactions such as constipation, nausea/vomiting, flatulence, and abdominal pain
- Dizziness, lightheadedness, and feeling faint
- Paraesthesia
- Somnolence, insomnia, and vertigo
- Reversible mental confusion, agitation, depression, and hallucinations
- Arthritic and myalgic symptoms
- Blurred vision, taste disturbance, peripheral oedema, increased sweating, gynaecomastia, and leucopenia
- Thrombocytopenia, agranulocytosis, pancytopenia, anaphylactic shock, malaise, and fever
- Bronchospasm, encephalopathy, hepatitis, interstitial nephritis, and hepatic failure
- Increases in liver enzymes
Pregnancy & Lactation
- No adverse effects on pregnancy or the health of the fetus/newborn child
- Can be used during pregnancy
- No information available on the passage into breast milk or its effects on the neonate
- Breastfeeding discontinuation recommended if essential use is considered
- Safety and effectiveness not established in pediatric patients less than 18 years of age
Precautions & Warnings
- Avoid concomitant use with clopidogrel
- Increased risk for osteoporosis-related fractures
- Atrophic gastritis in patients treated long-term
- Concomitant use with methotrexate may lead to toxicities
