Indications

  • Gastric and duodenal ulcers
  • NSAID-associated duodenal and gastric ulcers
  • Prophylaxis in patients with a history of NSAID-associated duodenal and gastric ulcers

  • Acid-related dyspepsia

Pharmacology

  • Inhibitor of gastric acid secretion
  • Blocks hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system
  • Onset of antisecretory effect within one hour
  • Maximum effect within two hours
  • Inhibition of secretion lasts up to 72 hours

Dosage

  • 20 mg once daily for benign gastric and duodenal ulcers
  • 40 mg once daily in severe or recurrent cases
  • Maintenance dose for recurrent duodenal ulcer is 20 mg once daily
  • 20 mg once daily for 4-8 weeks in gastro-esophageal reflux disease
  • 10-20 mg daily for long-term management of acid reflux disease
  • 10-20 mg once daily for 2-4 weeks in acid-related dyspepsia
  • 40 mg on the preceding evening, then 40 mg 2-6 hours before surgery for prophylaxis of acid aspiration
  • 20 mg twice daily for helicobacter pylori eradication regimen in peptic ulcer disease

Administration

  • IV Injection: Given slowly over a period of 5 minutes as prophylaxis of acid aspiration
  • IV Infusion: Given over a period of 20-30 minutes or more

Interaction

  • Decreased intragastric acidity may reduce the absorption of ketoconazole
  • Metabolized in the liver through cytochrome P450
  • May delay the elimination of diazepam, phenytoin, and warfarin
  • Plasma concentrations of Romilok and clarithromycin are increased during concomitant administration

Contraindications

  • Known hypersensitivity to Omeprazole
  • Suspicion of gastric ulcer while malignancy is not excluded

Side Effects

  • Skin rash, urticaria, pruritus
  • Photosensitivity, bullous eruption, erythema multiforme
  • Diarrhea, headache
  • Constipation, nausea/vomiting, flatulence, abdominal pain
  • Dry mouth, stomatitis, candidiasis

Pregnancy & Lactation

  • Results from studies indicate no adverse effects on pregnancy or on the health of the fetus/newborn child
  • No information available on the passage into breast milk or its effects on the neonate

Precautions & Warnings

  • Avoid concomitant use of clopidogrel and Romilok
  • Increased risk for osteoporosis-related fractures
  • Atrophic gastritis noted occasionally in gastric corpus biopsies from long-term use
